The information is as initially received without verification or evaluation, and is basically all that is known by the Region III staff on this date.

SUBJECT:

SCRAM AT PERRY NUCLEAR POWER STATION AND FEEDWATER LOSS (UPDATE)

DESCRIPTION:

On November 28, 2007, the Perry Nuclear Power Station automatically shut down as a result of feedwater control system problems. Coincident with the shut down, the plant experienced a loss of both turbine-driven feedwater pumps and the motor-driven feedwater pump. In addition the reactor core isolation cooling (RCIC) system failed to function as designed on two occasions.

Proper water levels were maintained in the reactor using the high pressure core spray system and, subsequently, the RCIC system.

A Special Inspection Team, with members from Region III (Chicago) and the Office of Nuclear Reactor Regulation, was dispatched to evaluate the event, the potential equipment deficiencies, and the licensees response to the event.

Based on direct inspection activities, the Special Inspection Team has concurred with the licensees determination that the cause of the automatic shut down originated in two power supplies in the feedwater control system. In addition, the Team concluded that the turbine-driven feedwater pumps operated as designed, based on the inputs from the feedwater control system. The Team also concluded that the licensees causal analysis for the RCIC system malfunction was accurate.

The special inspection has reviewed the licensees corrective actions to address the issues identified during the post scram review as requiring corrective action prior to startup. The Special Inspection Team, in conjunction with Regional Management has concluded that the actions taken to date are sufficient to allow startup.

The licensee is preparing to restart the unit. Both of the NRC resident inspectors will observe portions of the plant restart. In addition to areas normally observed during start ups, the residents will observe digital feedwater control performance, the motor driven feedwater pump operation; and testing for operability of the Reactor Core Isolation Cooling system.

The information in this updated Preliminary Notification has been reviewed with licensee management. The State of Ohio will be notified.
