Abstract

On March 8, 2022, a fuel oil tanker truck delivered a new fuel oil shipment of diesel fuel oil to Calvert Cliffs Nuclear Power Plant. As required per station procedure, samples were taken from the fuel oil tanker, with those necessary parameters verified within specification, prior to adding fuel oil to the fuel oil storage tanks. Other parameters including fuel oil lubricity, may be analyzed after fuel oil is added to the storage tanks. The analysis for those parameters must be received within 31 days following the initial fuel oil sample. Sample results for the new fuel oil delivered on March 8, 2022, which were received on March 25, 2022, documented a lubricity value of 614 micro-meters, which is above the acceptance criteria of less than 520 micro-meters. However, an issue report was not generated, and the Operations Department was not notified; therefore, the required technical specification actions were not taken within the required action completion time. This represents a condition prohibited by the plants technical specifications. The fuel oil in the fuel oil storage tanks was subsequently sampled and analyzed, with lubricity determined to be within the acceptable limit.

9/29/2023: CCNPP received NRC Inspection Report documenting Green Finding/Non-Cited Violation for failure to sample stored fuel oil to ensure Diesel Generator (DG) Technical Specification DFO requirements are met.

D. CAUSE OF EVENT

The method of discovery for this event was NRC identified and is documented in the sites Corrective Action Program under IR04706486. The direct cause of the event was failure to generate an issue report and failure to notify the Operations Shift Manager for the diesel fuel oil lubricity out of specification condition.

E. SAFETY ANALYSIS

As discussed above, Action Level 2 values require notification of the Operations Shift Manager and initiation of an issue report so that corrective actions are taken per applicable Technical Specifications, 3.8.3 Diesel Fuel Oil. Specifically, Technical Specification Limiting Condition for Operation 3.8.3, Condition E One or more DGs with new fuel oil properties not within limits, has a required action (E.1) to restore stored fuel oil properties to within limits within a Completion Time of 30 days. This required action was not completed within the 30 days required completion time. Technical Specification Limiting Condition for Operation 3.8.3, Condition F, Required Action and associated Completion not met, has a required action to declare the associated DG(s) inoperable, with an Immediate completion time. This required action was not completed within the required completion time. However, as discussed above, the subsequent test result of the stored fuel oil for lubricity was within acceptable limits, indicating the fuel oil remained within limits during the time of exposure.

The subject event satisfies the criteria in NUREG-1022, Revision 3, for any event or condition that was prohibited by the plants technical specifications. Therefore, this event is reportable pursuant to 10CFR 50.73(a)(2)(i)(B). This event did not result in any actual nuclear safety consequences.

F. CORRECTIVE ACTIONS

Calvert Cliffs personnel sampled the 1A FOST, and obtained the analysis to ensure lubricity was within acceptable limits.

G. PREVIOUS OCCURRENCES

A review of Calvert Cliffs events was performed. No events were identified from this review for any condition prohibited by the plants Technical Specifications due failure to sample diesel generator fuel oil storage tanks.
