License No. NPF-43 As a result of.the' inspection' conducted on Lar.e 26-30 and July.20, 1989, certain of your activities appear to deviate from your written commitment described in a lettet dated October 14,.1986, as. indicated below:

'Section (c)(2) ofc10 CFR Part 55.59, requires the requalification program to include preplanned lectures on a regular and continuing basis throughout the

< license period .for. normal, abnormal and emergency operating procedures.

By. licensee letter; dated October 14, 1986, the licensee indicated that procedures had been developed and implemented, and annual training would be performed for operators on the. dedicated safe shutdown panel (3L Panel).

License Condition 2.C.9(d) required the 3L Panel to be operational by December 31, 1986. Therefore, shift operators were required-to be retrained on the 3L-: Pane 1'during 1986 prior to the dedicated safe shutdown' panel being made operational.

Contrary to'the above, the licensee did not implement the annual requalification program for the 3L Panel during 1986, and 1988 (except for Shift 5 in -1988).

Please provide in writing within 30 days of the date of this Notice: (1) the reason'for the deviation; (2) the corrective actions that have been taken and

'the results achieved; (3) the corrective actions that will be taken to avoid further deviations; and (4) the date when your corrective actions will be completed. ' Consideration may be given to extending the response time for good cause'shown.
