In support of the request for renewal of the Special Nuclear Material License No. SNM-1305, the following additional information is submitted.
LicenseSNM-1305 currently authorizes possession and use of up to 30 Medtronic Model 9000 units, 60 Coratomic Model C-100 or C-101 units and 20 Cordis Omni-Stanicor Model 181.A nuclear-powered cardiac pacemakers.
To date, 23 Medtronic Model 9000 units are in use, 4 Coratomic units have been implanted and no Cordis units have been used.
Both Dr. Chardack and Dr. Gage are participating in the clinical evaluation m
of nucleta-powered cardiac pacemakers. Additional physician to be added to this license is Dr. Joginder N. Bhayana.
Dr. Jogirder N. Bhayana is the Chief of Thoracic Service at the Veterans Administration Hospital, Buffalo, New York, and Associate Profesoor of Surgery, State University of New York at Buffalo, Buffalo, New York. He received his MD degree in 1955 from MGM Medical College (Agra University),
Indore, India, and is licensed by the State of New York, No. 1149887.
Dr. Bhayana has been certified by the American Board of Surgery in 1964 and by the American Board of Thoracic Surgery in 1965.
Dr. Bhayana is familiar with the Medtronic, Coratomic and Cordia protocols and understands that they must be followed.
