Dear Mr. Malaro:
As indicated in the enclosed follow-up report, we have now used nine (9) Medtronic-L2urens-Alcatel nuclear powered cardiac pacemakers in nine (9) patients and we expect to implant the tenth unit in the very near future on a patient who is in the process of being transferred to this hospital from another Veterans Adminis-tration institution. We have in our hands requests to treat with this device several additional suitable patients.
W e would like to request from your office an extension and amendment of our License No. SNM-1305 to authorize the implantation in humans of an additional ten (10) nuclear pow ered cardiac pacemakers.
