Dear Mr. Singer:
The Buffalo Veterans Administration Hospital, Buffalo, New York, is licensed to implant nuclear powered pacemakers under license number SNM-1305.
It is requested that our license be amended to permit inclusion of the Cordis nuclear Omni-Stanicor, Model 184A, catalogue 307-C05.
We will follow explicitly the master protocol dated October 30, 1974 provided by the Cordis Corporation and approved by your offico. You can be assured that all institutional amendments previously submitttd are binding.
