Dear Mr. Mason:

Regarding the application for license application, the Buff alo Veterans Administration Hospital hereby requests amendment to its special materials license No SNM-1305 (Docket No. 70-1331) to implant a maximum of 60 Caratomic pacemakers annually. The Buffalo Veterans Administration Hospital agrees to follow the protocol.

Copies of the protocol have been provided to the Buff alo Veterans Administration Hospital and are on file. Staff physicians will be participating in this study.
