Subj: Special Duclear Material License SIU-1305 It is requested that the Specini Nuclear Paterial License No. SIIM-1305 be renewed.
This license expires on June 30, 1977, utilizing the Medtronic, Inc., IIodel 9000 nuclear powered pacemaker.-
Twenty-six nuclear pacenakers, Model 9000, have been used under this license. Two of the patients have expired and the units returned to Medtronic for evaluation. One has been removed and replaced with a Cordis, Model 190A, pace-maker. This leaves a total of 23 nuclear powered pacemakers in use under this license.
