2.0 GENERAL 2.1 The intent of the Control Room Panel Deficiency List and the Control Rod Drive Performance Log is:

To and the Operators in safe and efficient plant operation To aid Operators in the tracking and resolution of equipment deficiencies.

2.2 The Control Room Panel Doficiency List and The Control Rod Drive Performance Log are maintained in the Control Room.

2.3 This OM's data is maintained either manually or via computer.

2.4 U1-WC1 provides guidance for the disposition of deficiencies.

2.5 WP 8002 providas guidance for TR processing.

3.1 The following are examples of Control Room Panel deficiencies:

A component which is providing a faulty input to significant indications (lights, meters, gages, etc.).

An Inoperable instrument on a remote panel that prevents a Control Room alarm from functioning properly.

A plant component which has a condition, such as high vibration, such that it should only be used if attemate equipment is not available.

Control Room switch which does not return to normalif so designed.

3.2 The following are examplet af detidncies that would normally NOT be considered Contrcl Room Panel Deficiencios:.

Turbine 8th stage extraction steam pressure indication LPRM failures when bypassed (until the channel becomes Inoperable) e Packing and/or oilleaks that do not affect pump or valve OPERABILITY Vibration readings that do not affect component OPERABILITY e

3.3 Control Room Panel Deficiencies may be identified using TR stickers or labels.

1. The TR stickers or labels will be affixed to magnetic strips when possible, to minimize the fouling of control panel surfaces due to sticker residue.

- 2. The TR stickers or labels will be placed as close as possible to the deficiency

]f that it identifies.

3. The deficiency's information should be entered in the
  • Control Room Panel / Annunciator Deficiency List" Attachment 1.

ENTER the completion date for each deficiency resolved in the " Control Room Panet/ Annunciator Deficiency List' Attachment 1 REMOVE the TR sticker or label 4.0 CONTROL ROOM ANNUNCIATOR DEFICIENCIES 4.1 Control Room Annunciator Deficiencies may be identified using COLORED DOTS.

Each Annunciator COLORED DOT should be placed so as not to obscure the Annunciator's window's alarm title.

Print hard copies of Trouble Reports submitted against annunciator windows and insert into the approplate Alarm Response Procedure binder.

The printed trouble reports should be put into the ARP as the ' facing"(left hand) page so it is visible when referring to the ARP.

4.2.

As each deficiency is resolved:

REMOVE the annunciator's COLORED DOT from the annunciator window REMOVE the hard copy from its ARP e

ENTER the completion date for each deficiency resolved in the

  • Control Room e

Panet/ Annunciator Deficiency List" Attachment 1 5.0 CONTROL ROD DRIVE PERFORMANCE LOG 5.1 The Control Rod Drive Performance Log is intended to document equipment problems directly related to control rods.

5.2 Control Rod Drive deficiencies will be placed into one of two catagories:

5.2,1 Deficiencies which affect the operation of the control rod, and should be reviewed by the Operator PRIOR to manipulating the rod (Operationally Significant).

1. Examples:

Greater (San 300 psig d/p drive water pressure required to move a rod Double clutching needed to move a rod Rod overtravels Position Indication problems, i.e. no position 48, requires substitute rod e

position Excessive rod speeds, i.e. double notching e

Rod fails to latch at a particular position e

2. These deficiencies will be identified by:

PLACING a COLORED DOT on the control rod's pushbutton on the

" ROD SELECT MATRIX" ENTERING the information in Attachment 2," Control Rod Drive Performance Log" for each affected CRD PLACING a COLORED DOT in the

  • Operationally Significant" column in the
  • Control Rod Drive Performance Log" 5.2.2 Deficiencies which do not impact normal operation, but are recorded for trending purposes
1. Examples:

High accumulator water level Low nitrogen pressure High drive temperatures HCU valve performance problems Requires 260-499# d/p drive water pressure to move a rod Packing or fitting leaks

2. These deficiencies will be entered in Attachment 2, " Control Rod Drive Performance Log" for each affected CRD 5.2.3 For deficiencies which have been resolved:

REMOVE the COLORED DOT from the " ROD SELECT MATRIX" V

ENTER the completion date for each deficiency resolved in the " Control Rod Drive Performance Log" Attachment 2 6.0 DEFICIENCY LIST UPDATE 6.1 The updates shall be performed periodically as specified by the Unit 1 Operations Manager.

6.1.1 The update should review:

The Control Room Panel Deficiencies The Control Room Annunciator Deficiencies The Control Rod Drive Performance Log.

6.1.2 The computer copies of the " Control Room Panet/ Annunciator Deficiency List' Attachment 1 and the " Control Rod Drive Performance Log" Attachment -

2 should be updated by:

Entering all handwritten entries into the computer Delete all entries which have been resolved Forwarding the completed computer list to the Operations Manager 7.0 ATTACHMENT-Attachment i 1-OM 10.31 Control Room Panel / Annunciator Deficiency 1-OM-10.3 2 Control Rod Drive Performance Log s
