DELAVAL TURBINE COMPANY AND THERMEXCHANGER COMPANY MANUFACWRING INCONSISTENCIES (AITS F26050Hl)

During routine inspections conducted at WA's Bellefonte Nuclear Plant the RII inspectors examined " Inspection and Testing Branch" (I&T) inspection reports pertaining to Delaval's contract 76K61-86181 - M/O~ /

" Driven Emergency Generator Power Package".

Thermexchange M a sub-contractor for Delaval who provides N-stamped vessels used in the subject generator package.

The following extracts taken from the subject IST inspection reports appear to be matters that RIV's Vendor Inspection Program should follow-up on and may be of interest to you in the assessment of ASME third party inspection performance.

1.

Enclosure (1), Report 9, dated April 25, 1977, page 2 of 3 indicates that the TVA inspector witnessed the hydrostatic testing of lube oil coolers and jacket water cooler units that were code stamped and for-which code papers were signed by a California State Code Inspector prior to the units having completed satisfactory testing.

2.

Enclosure (2), Report 30, dated April 29, 1977, page 2 of 2 denotes that gap or weepholes were required by code in the stiffener ring on the air receiver tanks that were being hydrostatically tested.

None were provided, however, and these units had been bought off by the State Code Inspector and stamped with the class 3 "N" stamp.

Enclosure (3), Report 24, dated February 13, 19'2, page 2 of 3 disclosed that the WA inspector had noticed td)es that had

not been trimmed and deburred on a jacket water cooler assembly prior to hydro. The WA inspector was advised that this would be done af ter hydro and therefore the inspecto' declined to witness the hydro of the cooler until all work on the tubes was complete.

Enclosure' (5), Report 41, dated September 28,1978 on the bottom,

of page 1 alludes to possible doctoring-of radiograph reports.

RII' has addressed the subject Bellefonte Nuclear -Plant Contract as an open unresolved item and will follow it to a satisfactory resolution. Discussion conducted with RIV Chief, Vendor Inspection Branch disclosed that neither Delaval or Thermex-changer Companies have been inspected or are on a list.to be inspected.

In light of the above TVA. vendor inspection program report comments,' RII feels that the RIV Vendor Inspection Branch should look into the effectiveness of Delavals and i

Thermexchanger's established QA programs; for other possible generic implications to other nuclear contracts; and assessment of ASME third party inspection at the plants.

Uance that units had been stamped on initial inspection prior to bundle insertion.

Writer vill return to the Oakland area the week of h-25-77 for additional' inforr.ation and inspection.

i internal leaks when put on tubeside hydro has been re-rolled.. Lube oil cooler was tected id. 300 PSI and held for one hour, with no leakage or drop of pressure noted.

However, writer noted that a ctiffener ring has been cordtinuously velded 360 degrees. These velds and stiffene8 ring cross over a long seem veld which is subject to hydrostatic test. No gap or veephole vere provided on either this unit or on unit reported in Report No. 9,detG1 h-25-71 (S/N X121*bl) by v'riter as being hydrostatically tested and satistsetory.

Writer infomed Mr.' Nance that this ves unacceptable and that either a gap or weepho:

INSFECTION: Visual inspection vas perfomed for overall appearance and found to be satisfaetory.

CH PPIIIG: ?lo release for shipment was given at this time due to several questions which had not been ensvered and parts lists for shipment not available.

The questions were pertaining to drawings, acceptability of analysis of

. oil in jocket veter cooler. Inspector was in contact by telephone to TVA Q.C. end Engineerins Personnel in }~noxville and was advised that drevings ve e approved and analysis acceptable. Writer vill issue release on 4,.. n.n 2-7-78, when list is received at Pomona TVA Regional Office.

a Fetruary 2,1978 - Writer ves requested to be et/ Theme 2changeho witness the hydrona-ic test of jacket veter coolers which had been scheduled to test the previous day. Writer met with Mr. Denny Hollison, Q.C. Inspector for Thermexchanger. Jacket veter cooler, S/N X1ElT3B1 was at hydro test pressure of 323 PSI for shell side.

Writer recorded tirce of h:n3ro test start and proceded to make visual and dimensional inspection using drawings:

E-1209, Rev. 5, Channel Shell and Bonnet Details D-1864, Rev.1, Channel Nozzle Assembly D-1855, Rev.1, Shell Nozzle Stiffener Visual and Dimensional inspections were saticfactory on the shell and chonnels.

Writer declined to witness hydro of cooler until all work on tubes is completed.

Additional visual inspection of tubes on cooler being hydrostatic tested disclosed that several tubes around.the periphery of tubesheet had not been deburred. Writer advised Mr. Hollison that hydro test vould be acceptable only if hand files were us ed to deburr instead of the high speed drill and deburring tool normally used.

Mr. Hallison agreed t$ use hand file. Writer was advised that no further hydros vould k

be available until all vork is completed on tubes. Hydroe are scheduled for week of F-6-78.

To clarify Eeport 410, dated k-Fo-77, in which writer states that hydro tests of Air Receiver sbnks, S/N's X12172Al and X12172A2, vere rejected due to veep holes being critted in Stiffener rings. Weep holes were drilled into the etiffeners and tanks were re-hydrostatic tested, on 5-3-77, es reported on Report No.11, dated 5-3-77.

Writer vill return to Delavel, veek of 2-6-78, when additional material vill be ready for final inspection and hydro tests vill be witnessed at ThemexchanEer. Writer ses also advised that electrical panels at Delta Switchboard in Stockton, California, r.sy be ready for final inspection at this time.

QA Menener, stated that he would get TVA space but he has failed to make' but very little effort.

Inspection Report was issued on March 4,1980 (No. 99900700/E 75-01). Thermexchabger Incorporated (TX) manufacturers heat exchangers and is a prime supplier to the Transamerica Delaval Corp.
