Description:

Mixing Basin Discharge (001)

Outfall Code No:

800095 Parameter: pH Parameter Code No: 00400 1.

Date of Incident: December 1, 1985 0001-0800 hours 2.

Identification of specific effluent limitation or monitoring requirement affected:

Failure to monitor pH.

Description of Incident:

Grab samples of the mixing basin pH are required once per 8-hour shift when the monitoring or recording equipment is out of service.

On November,30 the mixing basin was last sampled at 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br /> and was not sampled again until 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br /> on December 1.

Misced sample was not discovered until MOR Form was filled out on January 7, 1986.

Apparent Cause:

Technician was not aware of sampling requirement.

taken to reduce, eliminate and prevent recurrence:

Remind technicians of sampling requirement when the sample pump is out of service.

released to the mixing basin between those hours which could have affected i

the mixing basin pH.
