Parameter: Oil and Grease Parameter Code Ko: 00556 1.
Date of Incident: March 23, 1986 (1000 Hours) 2.
Identification of specific effluent limitation or monitoring requirement affected:
Exceeded daily saximum limit 20 mg/L oil and grease 3.
Description of Incident:
Routine sample 'of the turbine sump oil separator tank (T-41) on 3/23/86 (1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br />) was reported on 3/27/86 to be 30.6 mg/L.

Apparent Cause:
Failure of oil removal system to remove oil from process stream.

If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
System engineer is in the process of evaluation.

Additional Comments:
A visual check of the mixing basin discharge showed no visible oil in the discharge. A sample collected the next day (3/24/86) from T-41 measured 1.3 mg/1. A sample collected the day previous (3-22-86) measured 2.8 mg/1.
