Description:

Mixing Basin Discharge (001) Outfall Code No: 800095 Neutralizer Tank Wastes (00E) 800213 Parameter: pH, TSS, Flow Vol Parameter Code No: 00400 00530, 50050

1. Date of Incident: February 7, 1986 1415-1500 hours
2. Identification of specific effluent limitation or monitoring requirement affected:

pH daily maximum limitation of 9.0 for mixing basin discharge.

Failure to monitor TSS and flow volume for neutralizer tank discharge.

3. Description of Incident:

On 2/7/86 a make-up water demineralizer was regenerated. The regeneration wastewater was routed to the neutralizer tank. Through an apparent over-sight, the neutralizer drain valve had not been closed following a previous release. The regeneration wastes, therefore, passed immediately through the neutralizer tank without sampling to the mixing basin and on to Lake Michi-gan. The mixing basin pH was elevated to 9.4 for 45 minutes.

4. Apparent Cause:

The neutralizer tank drain valve NV-DP1096 was left open following a previ-ous neutralizer tank release.

5. If not immediately corrected, the anticipated time the condition is expected to continue:

6. Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

The drain valve was closed at 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br /> on 2/7/86. The procedures for treating and releasing demineralizer regenerants will be reevaluated.
