SUMMARY

- CABLE SEPARATION Licensee: Carolina Power and Light Company (CP&L)

SUBJECT:

PROBLEMS ASSOCIATED WITH ELECTRICAL CABLE SEPARATION A meeting was held in the NRC Region II office on August 29, 1986, to discuss the status of CP&L actions relating to the resolution of an electrical cable separation problem at the Harris site. A previous meeting on this topic was conducted in the Region II office on July 30, 1986.

Region II began the meeting by stating that the main concerns appeared to be the determination or the separation problem root cause, correction of the problems and the verification that other discipline areas were not similarly affected.

CP&L discussed in detail, through a fault tree analysis approach, the apparent root cause. The basic root cause was the construction craf t generating a large number of separation deficiencies, coupled with the inability of QA to identify the overall problem in order to effect a solution to the craft problem. Both these problem areas were discussed in detail, describing sub-tiered causes for each major failure.

CP&L also discussed preventive and corrective actions planned and in progress to correct the cable separation problem. These actions included cable routing improvements, craf t and QC training, work flow and procedure improvements, and closer engineering involvement in deviation identification and correction.

The program scope and status were also discussed.

With respect to other discipline areas, CP&L stated that a similar fault tree analysis had been conducted on all other major construction activities. Using this approach, CP&L analyzed each activity with respect to several parameters in order to determine the probability of problem areas existing within the fault tree. Although the parameters were mainly objective, i.e., NCRs, SALP, inspection programs, the pronability assessment was primarily a subjective interpretation of the parameters. Based on the presented information, CP&L concluded that the root cause had been fully isolated and identified, that a sound corrective and preven-tive program was underway and that sound bases support the contention that the separation problem is isciated and not applicable to other areas.

Af ter a separate internal discussion, Region II concluded the meeting with a final discussion. Region II believes that CP&L has control over the cable separation problem. SEP 161986 Enclosure 1 2 actions. In order to objectively verify the adequacy of the fault tree analysis for the other construction activities, Region II will conduct an inspection during the week of September 1, 1986, into selected activities.

On September 3, a telephone conversation was conducted between A. Watson, CP&L and R. Walker, Region II. During this call, CP&L was informed that the fault tree inspection would be conducted from September 4-11. The specific areas to be inspected are structural steel and electrical cable tray supports.
