Thank you for your letters of January 31,1986(85-28) and February 21, 1986 (85-33), in response to our letters and Notice of Violations (NOVs) dated January 2, 1986 (85-28), and January 29, 1986 (85-33). Although you have indicated that portions of violation 8528-01 are not valid concerns in this case, it appears to us that the errors of omission that you cite as caused by inattention to detail were actually caused by a confusing condition identification work authorization (CIWA) system that resulted in your employees not knowing how to properly fill out the-CIWA form.

Your proposed corrective actions appear to address this larger concern. We have reviewed your replies and find them responsive to the concerns raised in our NOVs.

We will review the implementation of your corrective actions on the above NOVs during a future inspection to determine that full compliance has been achieved and will be maintained.

If you have any questions, we will be pleased to discuss them with you.
