DISCUSSION WITH MR BILLY RAY JONEC REGARDING ALLEGATIONS OF WORK PRACTICE AT' COMANCHE PEAK STEAM ELECTRIC STATION On January 26, 1984, in company with Brooks H. Griffin, I interviewed 4. Jones concerning certain allegations of poor work practices. The following informatton ~

was obtained regarding the six items in the January 6, 1984, memorandum.

1. MS-1-004-007-C72K Shock Suppressor and Restraint Excessive gap is located on the opposite side of etching that was performed. The fit up was correct in that location, but enlarged as it proceeded toward the wall. The bottom kicker is correct location.
2. MS-1-001-903-C77W Pipe Whip Restraint There is no problem with the whip restraint (U-bolt) itself. There is a problem with the structure supporting the restraint. The web was cut out in the wrong location. Instead of reporting and repairing according to

.s procedure, it was filled in by welding. Should talk to Williamson on this. (See Generic Traveler MS-1-001-901-C77W, Pipe Whip Restraint and SupportInstallation,M-25,SI-0585)

3. MS-1-003-009-C72K Shock Suppressor The stantions were welded inside with "he11 arc" (welder was " Rick" ?).

It was backwelded because of excessive gap. The covering plates were not -

on at that time. The pipe stantion was punch marked bylr. Jones and sent to fab shop to cut. Too much pipe was cut off at tne lower point.

9r. Messerly welded in and ground down the weld beads. Mr. Jones refused to have anytning to do with the weld and nonfit. This was on the top stantion.

4. MS-1-003-010-C72K Shock Suppressor .

The bottom saddle was cut in four pieces. The left hand, back piece did not fit due to change in pipe bend. It was heated with rosebuds to cherry red. A 20 ton jack was used, supported by 4" x 6" timbers to bend metal. A 3 1/2 ton come-along was also used. This was done under direct orders of.H. Heabert, Superintendent, Ed Dean, General Foreman, CARDE84-779 PDR -

4 A covering was put around the area to hide the bending operation. People j were posted to watch for QC personnel. The entire saddle weld was ,

stress relieved.

5. MIS-1-003-903-C77W Pipe Bumper
This was the wrong item. MS-1-002-005-C72K was identified as the correct support. There is a gap in the steel that made up the support box. [

. Stanley Williamson and Mr. Jonet went to the superintendent and reported j st. Nothing was done.

6. FW-1-017-712-C72K FW Pipe Support j Nothing wrong - only mentioned as one that Mr. Jones had worked on
previously.
