SUBJECT:

ALLEGATIONS CONCERNING WORK PRACTICES AT COMANCHE PEAK (BILLY RAY JONES)

On January 6,198?, I accompanied Mr. John Collins, Regional Administrator, Regien IV, and NRC Inspector, J. Cummins, during a site visit with Mr. Billy Ray Jones. Mr. Jones had alleged poor work practices at Comanche Peak.

The following specific items of concern to him were identified during the visit. His concerns were related to the main steam system pipe supports, pipe whip restraint, stantions and associated saddles. One item on the main feed system was also identified.

1. MS-1-004-007-C72K Shock Suppressor Excessive gap in the 45 angle of restraint - the bottom kicker and outrigger gap welded up.
2. MS-1-001-903-C77W Pipe Whip Restraint Whip restraint was damaged and repaired'without a procedure. Gouged out on bottom by foreman. Stanley Williamson worked on restraint.

Mr. Messerly welded on it without a procedure. Restraint is on Loop 1 MS pipe.

3. MS-1-003-009-C72K Shock Suppressor Stantion buttered up with weld bead after cutting excess of 1" of metal off. Joe Ray was welding foreman. Bob Messerly helped; Raymond Hebert told him to do it. No NCR was generated. Top station "ok," bottom station had 1" wide weld.
4. MS-1-003-010-C72K Shock Suppressor Saddle cinched down with Portapower and heated to bend pipe to fit MS line. Saddle did not fit radius of pipe. Was heated to cherry red; no procedure used; no temp sticks used to check temperature. Before QC was called to inspect welds all burn marks were buffed off and removed.

Bob Messerly and Ed Dean were involved in this. No stress relieving done; dye penetrant check done.

5. MS-1-003-903-C77W Pipe Eurcer Ecessive gap on plates (shims) welded over.

Stanley Williamson, worked with Mr. Jones, went to superintendent about problems (now living in Joshua, Texas) Has left the site. Told Herbert, Sonny Rinner (Supt Hanger Dept.) about concerns.
