NRC Fonn 540 must be completed for shipments oflow-level radioactive waste intended for uhimate disposal at a licensed land disposal facility. The majority of the infonnation on NRC Fonn 540 is needed to meet DOT shipping paper requirements for radioactive material

shipments. The following are general instructions. Detasi regarding DOT requirements can be found in 49 CFR Part 172.
1. Emergency Telephone Number - The shipper is to provide an emergency response telephone number or numbers for use in the event of an emergency. If a single number applies to every entry on the shipping papers, enter this number in the space provided. If there are multiple numbers, each individual number must be entered in item i 1 immediately following the applicable description of the radioactive material.
