adequate protective measures can and will be taken in the event of a radiological emergency. This PID thus disposes of all remainir,g issues admitted for litigation and i

resolves them in favor of the applicant except for any issue which may arise from the l

inmates of the State Correctional Institution at Graterford, Pa.

V. C. Sumer Nuclear Station

..The."A" and,"B" diesel generators were declared inoperable on May 8,1985 while testing i

in accordance with technical specifications. The voltage regulator did not work properly 1

on the "A" diesel generator and a noise was heard and water observed on the side of the j*

"B" diesel generator. Water was found inside the il cylinder. The Plant was in hot standby (mode 3) at the time.

On May 11,1985, for the second time in 21s months, V.C. Summer achieved criticality sooner than the calculated ECP predicted by greater than 100 steps and actual critical position did not meet the Technical Specification rod insertion limit. The plant was i

shutdown by emergency boration. The licensee reviewed with W the ECP calculation and found no problems and the plant was satisfactorily started uF on May 12, 1985.

j Nine Mile Point Nuclear Station. Unit 1 l

On May 7 and 8,1985, thru-wall cracks were discovered in two of the four containment

~

spray heat exchangers, and they were declared inoperable. Subsequently, visual and surface indications were identified in the two remaining containment spray heat exchangers. The facility is currently operating in a 7 day Limiting Condition for Operations (LCO) while evaluation of the remaining two heat exchangers continues.

The licensee has remedial repairs in progress to preclude any potential for gross structural structural failure of the heat exchangers.

l Three Mile Island. Unit 2 i

On May 15,1985, the TMI 2 plenum assembly (PA) was lifted from the reactor vessel and transferred to storage in the deep and of the fuel transfer canal. No significant increase in airborne contamination levels in the reactor building was observed. The safe removal and storage of the PA now allows GPU to complete the installation of t

equipment necessary for reactor defueling, currently scheduled to comence in Sept.1985.

WEEKLY INFORMATION REPORT OFFICE OF ADMINISTRATION May 13th, the Executive Director for Operations signed a Federal Register notice seeking public comment upon a proposed size determination standard developed by the Division of Rules and Records, ADH and approved by the Small Business Administration. The one million dollar size standard is proposed for use in identifying small entities for purposes of implementing the review of NRC regulations required by the Regulatory Flexibility Act of 1980.

Shoreham On May 7, 1985, the Shoreham Emergency Planning ASLB re-opened the hearing record concerning the suitability of-the Nassau County Coliseum as a relocation center. The Board order greatly limited the scope of the proceeding and scheduled the hearing to resume on June 4, 1985.

-Limerick On May 2, 1935 the Atomic Safety and Licensing Board issued a partial initial decision (PID) on offsite-emergency planning which concluded as follows:

The emergency plans, subject to conditions set forth in the order, meet the requirements of 10 CFR 50.47, and Appendix E to Part 50, as well as the criteria of NUREG-0654, and provide reasonable assurance that adequate protective measures can and will be taken in the event of a radiological. emergency.

-This PID thus disposes of all remaining issues admitted for litigation before the Licensing Board and resolves them in favor of the applicant except for any issue which may arise from the inmates of the State correctional Institution at Graterford, Pa.

V. C. Summer Nuclear Station j

On May 8, 1985 at 7:30 p.m., while testing the "A" diesel generator in accordance with Technical Specifications 3-day test frequency, the voltage regulator did not work properly and the "A" diesel generator was declared inoperable. Then at 7:50 p.m., while performing Technical Specification required surveillance on the "B" diesel generator, a noise was heard coming from the "B" diesel generator during its shutdown. Water was observed on the side of the "B" diesel generator and it was declared inoperable. Water has now been found inside the #1 cylinder. The plant was in hot standby (Mode 3) at the time.

Three Mile Island, Unit 2 The TMI Program Office (TMIPO) has approved the GPU Nuclear plan for removal of the THI-2 plenum assembly (PA). The PA, currently supported by jacks at a height 7-1/4 inches above its normal position, will be lifted out of the reactor vessel using the polar crane, tripod and specially designed lift rigging. The PA will be horizontally transported through air for approximately 32 feet, then lowered onto its storage stand in the deep end of the fuel transfer canal, where it will be shielded under 4 feet of water.

In its Safety Evaluation, dated May 7,1985, the staff concluded that:

(1) there is little potential for uncovering the damaged core or for recriticality during PA removal; (2) there is little potential for a load drop accident involving the PA and that adequate measures are available to mitigate the consequences of such an unlikely event; and (3) the licensee has instituted appropriate measures to maintain worker exposures ALARA during PA lift and transfer. The TMIPO staff and an expert consultant observed the operation of the special lifting equipment during mockup testing conducted on April 30, 1985.

The actual lift, transfer, and storage ofthe plenum assembly is scheduled to take place during the week of May 13, 1985, subject to completion of prerequisite activities and final NRC approval of the detailed procedures.

Nine Mile Point Nuclear Station, Unit No. 1 On May 7, 1985, during a routine surveillance test, the licensee discovered a thru-wall crack on the water box for one of the four containment spray heat exchangers used to cool torus water. After removing the paint, the crack was observed to be about 5 inches long in the shell to tube sheet weld. The licensee declared the heat exchanger inoperable and entered a 15 day Limiting Condition for Operations. On May 8, 1985 the licensee discovered a thru-wall crack and other surface indications (by dye penetrant (PT) testing) in a second containment spray heat exchanger, declared the heat exchanger inoperable and entered a 7 day LC0 (effective May 8, 1985).

Subsequently, visual and surface indications have been identified in the two remaining containment spray heat exchangers. All indications are located in the shell to tube sheet welds on the water box sides. The two remaining heat exchangers are under evaluation.

The facility is currently operating in a 7 day LC0 while the evaluation of the remaining two heat exchangers continues. Each heat exchanger has 100% cooling capacity. The licensee has remedial repairs in progress to preclude any potential for gross structural failure of the heat exchangers.

N OSURE B MAY 171985

V. C. Summer Nuclear Station For the second time in 21 months, V. C. Summer achieved criticality sooner than the calculated ECP predicted by greater than 100 steps (Target Band is plus or minus 50 steps). This latest occurrence was on Saturday, May 11, 1985. Also, for this startup, actual critical position did not meet the Technical Specification rod insertion limit.

The plant was shutdown by emergency boration in accordance with plant procedures. The licensee reviewed with Westinghouse the ECP calculation, and found no problems. The licensee started up the plant satisfactorily on Sunday, May 12, 1985.

After the first occurrence, the V. C. Sunner ECP procedure was upgraded to take xenon into better account.

Yankee-Nuclear Power Station LOCA Analysis In March,1985, Exxon Corporation informed the NRC of errors in Exxon

~

LOCA analysis codes.

Investigation by NRR with subsequent reanalysis by the licensee has revealed a limitation in the LOCA model used for the Yankee plant. The limitation could possibly cause analysis limits to be exceeded in the upper core region during core reflood if a LOCA were to occur late in the fuel cycle.

Based on reanalysis results available to date, the licensee has voluntarily taken action to reduce the flux in the upper core region until the analysis is completed.

Region I and NRR are following the reanalysis activity.

Grand Gulf, Unit 1 The plant achieved 100 percent power Sunday, May 12, 1985, at 8:00 a.m.

The plant is undergoing startup testing in the last phase, Test Condition 6, which ranges between 95 and 100 percent power. The plant was brought up to 100 percent power (1204 megawatts electric) for one hour to make measurements at that power level for the first time.

The licensee has scheduled completion of startup tests in July 1985.

There are 34 startup tests in Test Condition 6.

One of the tests, turbine trip and generator load rejection, will be a more severe transient than the planned test because only two of the three main steam bypass lines are in operation. The transient would be bounded by safety analyses, however, which are based on no steam bypass.

One main steam bypass line has been taken out of service because of a non-through wall crack in the piping downstream of the valve. The 6-inch longitudinal crack was found by ultrasonic inspection. The licensee is evaluating repair of the bypass line and operation with one bypass line closed to determine the needed repair and the optimum time to make the repair.

_4 Three Mile Island, Unit 2 On Wednesday, May 15, 1985, the TMI-2 plenum assembly (PA) was lifted froin its jacked position in the reactor vessel and lowered onto its storage stand in the deep end of the fuel transfer canal, where it is shielded under four feet of water. The reactor building polar crane, tripod, and lift rigging performed satisfactorily throughout the lift, transfer and storage sequence.

No significant increase in airborne contamination levels in the reactor building was observed during the PA removal. PLANT STATUS The facility remains in long term cold shutdown with the Reactor Coolant System (RCS) vented to the reactor building atmosphere and the reactor vessel head and plenum assembly removed.

RCS cooling is by natural heat loss to the reactor building ambient atmosphere.

Incore thermocouple readings range from 72*F to 92*F with an average of 81*F. Average cold leg tem>erature is 58'F.

Calculated reactor decay heat is less than 14 d10 watts.

- The modified internals indexing fixture is installed on the reactor vessel flange and is flooded to elevation 327 feet 6 inches (151 feet above the top of the core region).

The average reactor building temperature is 60*F. The reactor building airborne activity is 1.3 E-9 uC1/cc tritium, and 5.0 E-10 j

.uCi/cc particulates, predominantly cesiu;:137.

The reactor vessel plenum has been removed from the reactor vessel and placed on its storage stand in the deep end of the fuel transfer l

canal. A dam has been installed between the deep and shallow ends of the fuel transfer canal. The deep end is filled with water to a depth of about 20 feet.

WASTE MANAGEMENT The Submerged Demineralizer System (SDS) completed processing batch S-118, consisting of 12,217 gallons of miscellaneous waste from the neutralizer tanks.

No EPICOR II processing occurred this week.

. Total volume processed through SDS to date is 2,875.999 gallons, and the total volume processed through EPICOR II is 2,447,788 gallons.

ENVIRONMENTAL MONITORING

The Lancaster water sample taken at the water works intake and analyzed by the US Environmental Protection Agency consisted of a seven day composite sample taken from April 28, to May 4, 1985. A gama scan detected no reactor related radioactivity, i

the plant discharge to the river consisted of seven daily composite samples taken from April 27, to May 1,1985. Gama scans detected no reactor related radioactivity.

The NRC outdoor airborne particulate sampler at the TMI Site collected a sample between May 8, and May 16, 1985.

No reactor related radioactivity was detected. Analysis showed I-131 and Cs-137 concentrations to be less than the lower limits of detectability.

The above EPA sample analysis results show TMI. site liquid effluents to be in accordance with regulatory limits,,NRC requirements, and the City of Lancaster Agreement.

REACTOR BUILDING ACTIVITIES l

The plenum assembly.was removed from the reactor vesstl and transferred to storage in the deep end of the fuel transfer canal on May 15, 1985. The maximum measured dose rate, 36 feet directly below the plenum, was 80 R/hr. Radiation levels in the refueling canal returned to pre-transfer levels once the plenum was stored under 6 feet of water in the deep end of the canal.

Future work in the reactor building will be focused on preparations for the first phase of defueling in September 1985. The near term

.defueling preparations include installation of a 5-ton service crane over the refueling canal and completion of the Defueling Water Cleanup System.

AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES Installation of the Defueling Water Cleanup System (DWCS) continued.

Testing of the fuel transfer system and upenders has been completed in the 'A' spent fuel pool.

Boration of the processed water storage tank number 1 is in progress.

The chemistry of this water is being adjusted to provide a source of

' RCS quality water for flooding of the 'A' spent fuel pool. The f

The Panel received a presentation from representatives of General Public Utilities Nuclear Corporation on plans for reactor fuel removal and storage. The Department of Energy briefed the Panel on the current status of fuel shipping casks that will be used for offsite transport of fuel and debris removed from the reactor. The NRC staff provided the Panel with an update on the status of NRC investigations and enforcement actions. The date and location of the next Panel meeting will be announced in the future.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending May 17, 1985 Multi-Loop Integral System Test (MIST)

A Program Management Group (PMG) meeting was held on April 30-May 2,1985 in All_iance, Ohio'to discuss MIST construction and analysis issues. The PMG has accepted the Babcock and Wilcox (B&W) recommendation to use existing heater rods and braze techniques and replace the shorted heaters.

In addition B&W will also perform a feasibility study of lower temperature braze through braze alloy selection. There is no expected impact on the planned startup of the facility in September 1985.

The system modifications necessary to perform secondary system feedwater line breaks in Semiscale have been completed. System checkout is in progress and the initial attempt to perform the first feedwater line break test is May 22, 1985. The purpose of the feedwater line break test series is to provide additional data to assess the accuracy of the best estimate codes.for this type of event. Conservative analyses indicate that in certain cases feedwater line breaks can result in reactor coolant system overpressurization that exceeds Standard Review Plan limits, i.e.,

110 percent of system design pressure.

Lehigh University A review of the Post-Critical Heat Flux (CHF) Heat Transfer experiment at Lehigh University was held at NRC on May 6,1985, to provide the status of the program to NRR and RES staff. The purpose of the experiment is to provide data to assess post-CHF correlations used in nuclear reactor i

system codes, NRC LOFT A report entitled " Summary of the Nuclear Regulatory Commission's LOFT Program Research Findings " was published as NUREG/CR-3005. The report is a summary of the main research results of the Loss-of-Fluid-Test (LOFT) program (pre-0 ECD)relativetocodeassessment,codedevelopment, licensing, rulemaking, safety technology, and reactor operations.

2-Tests of Selected Electrical Circuits A series of tests of selected electrical circuits has been performed at the Shippingport Atomic Power Station as the first step in a continuing effort to gather data tnat may be useful in better under>Londiny (n-service agin; degradation of safety equipment at commercial LWRs. Shippingport, which was operated by DOE for the Nuclear Navy, is a PWR with many design features and conponents typical of a commercial LWR. Decomissioning activities at Shippingport are expected to begin later this year and, through cooperation with D0t, arrangements have been made to obtain equipment and coolant boundary sanales for examination and testing as part of the aging research program being tenducted by RES. The initial series of tests were conducted to assess the

~ tatus of selected electrical circuits before removal for additional analyses.

f ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING MAY 17, 1985 Arrangement Activity The. staff this week sent the following three agreements, which have all been cleared by the Department of State, to the EDO for signature:

1. The "Agreemcnt on the OECD Halden Reactor Project Covering the Period January 1, 1985 to December 31, 1987," which had been held up pending Halden's acceptance of several conditions on continued NRC participation which NRC had specified after undergoing significant research budget reductions.
h. The "NRC-Philippine Atomic Energy Commission (Renewal) Arrangement for the Exchange of Technical Infonnation and Cooperation in Nuclear Safety Matters," which extends for another five years the information exchange and cooperation activities currently underway between NRC and the PAEC.
3. An Addendum to the NRC-IAEA Memorandum of Understanding (Administrative) which details how the IAEA will reimburse (rather than prepay) NRC for expenses it incurs in supporting and participating in the IAEA's program of technical assistance to less-developed countries.

International Visitors On Monday Mr. Kjell Andersson and Mr. Alf Larsson of the Swedish Nuclear Power Inspectorate (SKI) and Mr. Inge Pierre of the Swedish Embassy in Washington met with technical representatives from NMSS and RES to discu::s several aspects of NRC's waste management activities. They were especially interested in NRC's strategy for licensing HLW repositories and NRC's approach to reviewing DOE's work on site characterization.

They also expressed an interest in hydrological modeling and perfonnance assessment of repositories. The visitors took this opportunity to note recent progress in the hydrocoin project headed by SKI, which is aimed at modeling nuclide transport behavior in geological formations. Before coming to the NRC the Swedish team also had an appointment at DOE to discuss similar topics.

The Court had directed in the tentative decision that either Party had 10 days to soecify controverted issues or make proposals not covered in the The State of California received an extension to the 10-day ifmit decision.

and on May 1, 1985, the State submitted a response specifying controverted issue and objections to the tentative decision. OSP has been infmned by California that Chem-Nuclear will have 20 days to submit a response to the Court on the issues raised in the State's response to the decisicn.

Panel on Price-Anderson in Chicago, Jerome Saltzman (OSP) participated in a panel On May 13, 1985, on Price-Anderson alternatives before a comittee of Governors' advisors on energy of the National Governors' Association. Representatives of the Congressicnal staff, electric utilities and public interest groups also participated in the panel.

OFFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEMS OF INTEREST WEEK ENDING MAY 17, 1985 1.

An Engineering Evaluation Report (AE0D/E506) was completed on the review of an event involving stem fracture of a valve in the RHR system. The valve stem was made of type 410 stainless steel. Metallurgical examination showed that the stem had failed from intergranular stress corrosion cracking (IGSCC). Excessive surface hardness caused by improper heat treatment during manufacture of the stem material was found to be a contributing factor to the stem failure.

In this engineering evaluation, a search of the LER data base was made which revealed that similar events have occurred at three other plants involving valves of different sizes and manufacturers. The valve stems in these events were also made of type 410 stainless steel. Valve stems made of 400 series stainless steel and heat treated to high hardness are highly susceptible to IGSCC. The excessive hardness is not detected in the usual licensee or supplier's QA programs. Also, the existing code requirements do not specifically address the actual testing to verify the hardness for the stem material after heat treatment.

Based on the results of this study AE0D suggested that IE consider issuing an Information Notice for feedback purposes and RES consider the adequacy of the applicable code requirements for assurance of proper heat treatment.

2.

Representatives from IE and AE00 gave a two day workshop to licensee personnel on the reporting requirements contained in 10 CFR 50.72 and 50.73. The work-shop was attended by 107 people representing 48 utilities with operating nuclear power plants. NRC The Commission by a 4-1 vote (with Commissioner Asselstine disapproving) approved a policy statement which provides interim guidance to the adjudicatory boards and the staff until the Commission can conclude its response to the D.C. Circuit Court's remand in Guard v. NRC.

The guidance states that "the Commission believes that Licensing Boards (and, in uncontested situstions, the staff) may find that applicants who have met the requirements of Section 50.47(b) (12) as interpreted by the Commission before the Guard decision and who commit to full compliance with the Commission's response to the Guard remand meet the requirements of Section 50.47 (c) (1) and, therefore, are entitled to licenses conditional on full compl'iance with the Commission's response to the Guard remand."
