PRELIMINARY NOTIFICATION May 4, 2020 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE - PNO-III-20-001 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. Some of the information may not yet be fully verified or evaluated and is basically all that is known by the Region III staff on this date.

UNPLANNED SHUTDOWN GREATER THAN 72 HOURS DUE TO REACTOR COOLANT SYSTEM LEAKAGE EXCEEDING TECHNICAL SPECIFICATION LIMITS On May 1, 2020, at 10:00 a.m. (all times EDT), with the plant at full power, operators at the Donald C. Cook Nuclear Plant commenced a controlled plant shutdown in response to reactor coolant system (RCS) unidentified leakage.
On May 1, 2020 at 3:54 a.m., control room operators received alarms indicating elevated containment radiation, humidity, temperature, and sump levels. Operators diagnosed unidentified reactor coolant system leakage of approximately 8 gallons per minute. The licensee entered Technical Specification (TS) 3.4.13 Limiting Condition of Operation (LCO)
Condition A for exceeding the unidentified leakage limit of 0.8 gallon per minute.
On May 1, 2020, at 9:45 a.m., licensee personnel entered Unit 2 containment and identified that the leak was from a pressurizer spray valve leak-off line, through a degraded cap. The licensee entered Technical Specifications 3.4.13 LCO Condition B for RCS leakage and will make necessary repairs before restart.
All other systems functioned normally. There was no impact on public health. Unit 1 remains at full power.
Region III received notification of this occurrence from the resident inspectors. The senior resident inspector responded to the site and monitored plant parameters and licensee actions to identify the leak source and shutdown and cooldown the plant. The NRC resident inspector staff, assisted by inspectors from the NRC Region 3 Office, continue to monitor licensee actions to identify the leak and continue to monitor the repair activities.
The State of Michigan has been notified.
