NOTICE OF UNUSUAL EVENT DURING PLANT SHUTDOWN DUE TO INCREASED REACTOR COOLANT SYSTEM LEAKAGE DESCRIPTION:

On September 13, 2017, at 12:47 a.m. EDT, Cook Unit 1 declared a Notice of Unusual Event (NOUE) after control room operators observed increasing reactor coolant leakage shortly after the plant had been shut down in preparation for a refueling outage. The leakage resulted from a pressure relief valve that had lifted shortly after the shutdown. This valve was located on a section of piping connected to the reactor coolant system and was intended to protect the piping integrity. The leakage from the valve was routed to a tank located inside the Unit 1 containment where it was contained as designed; therefore, there was no leakage of radioactive material to the environment. Subsequently, the licensee was able to isolate the affected piping thereby stopping the reactor coolant system leakage. Currently, D.C. Cook Unit 1 is shut down and stable. Unit 2 remained at 100 percent power operation during the event. The NOUE was exited at 8:00 a.m. EDT on September 13, 2017.

The NRC resident inspectors were in the control room during the shutdown and observed the licensee actions during and after the NOUE declaration. The NRC determined that this event posed no immediate safety or public health concern.

The licensee is investigating why the relief valve lifted. The licensee also visually inspected the affected Unit 1 piping and identified no issues. The NRC resident inspectors will continue to closely monitor this event, including the licensees investigation, and will conduct a thorough, independent inspection. The results of this inspection will be documented in a future inspection report.

NRC regulations require that the NRC be informed of any event or situation related to the health and safety of the public or protection of the environment for which an emergency declaration has been made.

The NRC was notified of this event on September 13, 2017, at about 1:00 a.m. EDT (reference NRC Event Notification Report 52967). The information presented herein has been discussed with the licensee and is current as of September 13, 2017, at 2:00 p.m. EDT. The State of Michigan was also notified of this event.
