D.C. COOK UNIT 2 MANUAL TRIP RESULTING IN A SHUTDOWN FOR GREATER THAN 72 HOURS On July 6, 2016, at 00:38 EDT operators manually tripped D.C. Cook Unit 2 following a loud noise in the turbine building and indications of a large steam leak. Shortly after, at 00:50 EDT, the licensee declared an Unusual Event, which is the lowest level of alert on the Nuclear Regulatory Commissions emergency response scale, for a rupture in the steam line with damage to permanent plant equipment. While there was damage to some nearby non-safety related equipment and to some of the exterior siding to the turbine building (no structural damage), there was no damage to any safety-related equipment. Additionally, the plant shut down was completed without incident.

As of July 11, 2016, the licensee had completed the necessary repairs to the steam line and to the turbine building and was making preparations to restart the plant. The rupture occurred after one of the expansion bellows on the steam piping failed. These bellows are used in systems that contain high temperature substances such as steam, to absorb movement and vibration. The affected bellows was replaced and the licensee inspected the other associated expansion bellows prior to commencing the restart of Unit 2. The NRC Resident Inspectors responded to the shutdown once notified and were monitoring the licensee repair and subsequent restart activities. The NRC Resident Inspectors continue to monitor the licensees investigation into the cause of the rupture.

This preliminary notification is issued for information only and no further action by the staff is anticipated.

Region III received initial notification of this occurrence by telephone call from the licensee at 01:02 EDT on July 6, 2016. The information presented herein has been discussed with the licensee, and is current as of 13:00 CDT on July 12, 2016. The State of Michigan has been notified of these events.
