SUBJECT:
RECOMMENDATION FOR NOTIFICATION OF LICENSING BOARD Enclosed is a potential 10 CFR 21 report from the Zack Company. The Zack Company is a heating, ventilation, and air conditioning (HVAC) subcontractor at three power plant construction sites within Region III (Clinton, LaSalle and Midland). The subject report discussed deficiencies concerning the accuracy of welder records and is limited to work performed at the Zack facility in Cicero Illinois. The Zack Quality Assurance (QA) program is an issue which will be addressed in the Midland OM/OL proceedings.
Region III has' reviewed the materiality and relevancy of this matter and perceives the infor:tation identified in the enclosure to be material and relevant to the Midland OM/0L proceedings. Region III believes NRC policy dictates that the information be forwarded to all sitting boards for cases involving Zack Compat.y as the HVAC subcontractor.
The Zack Company has initiated an investigation of the welder record discrepancies. Region III is presently waiting for a followup report from the Zack Company which will identify th& results of this investigation.
This followup report is expected by September 10, 1982.

RECOMMENDATION FOR NOTIFICATION OF LICENSING BOARD In accordance with present NRC procedures regarding Board Notifications, the following information is being provid.ed as constituting new information rele-vant and material to the Midland OM/0L proceedings. This information deals with the licensee's November 3,1983, decision to issue a stop work order pertaining to eight Project Quality Control Instructions (PQCI's) which control the in-spection of electrical instrumentation support welds.
The eight PQCI's contain duplicate welding criteria. The potential exists that if a PQCI was revised, then all other PQCI's having duplicate welding criteria may not be revised. As a result, inspections could be performed to superceded requirements. This issue was identified by Stone and Webster during their over-view of the Construction Completion Program (CCP).
No work was being performed which was affected by the stop work order, however, the order will prevent the licensee from initiating phase 1 of the CCP as it relates to the inspection of electrical instrumentation support welds until corrective actions have been taken and the stop work order rescinded.

This Notification is provided in accordance with NRC procedures regarding Board Notifications and is deemed to provide new infomation material and 4
relevant to quality assurance / quality control issues in the Midland OM-OL proceeding. As noted in Enclosure 1, Consumers Power Company (the Applicant) issued a stop work order on November 3,1983, be:cause of duplicate welding criteria contained within eight Project Quality Control Instructions (POCIs) used to control inspections of support welds for electrical instrumentation.
A conce:Ti arises from the potential that, if a POCI is revised, not all other PQCI:, with duplicate welding criteria may be revised accordingly.
In such a case, inspections could be perfomed to requirements which should have been superceded.
l Initiation of phase I of the Applicant's Construction Completion Program i
(CCP) with respect to inspection of electrical inspection support welds could be affected by this stop work order in the future. At present, however, no work is being, nor has been perfomed because of earlier stop sork orders reported by BN 83-167. The NRC staff and Stone and Webster, the third party overviewer of the CCP, will monitor the Applicant's corrective actions regard-ing the eight POCIs. The staff will report further to the Board as resolu-tions are achieved.

This Notification is provided for your infomation in accordance with NRC pro-cedures regarding Board Notifications and is deemed to provide new infomation material and relevant to quality assurance issues in the Midland OM/0L pro-ceeding. As stated in the enclosure, on October 5, 1983, Babcock and Wilcox (B&W) decided to stop all its seismic Class I pipe hanger and snubber activi-ties and lay off 132 craft and support personnel. The decision resulted from inspections by Bechtel and B&W which identified various discrepancies in the manufacture and installation of hangers and a number of deficiencies, including welding and bolting problems.
The NRC plans to follow the corrective actions which will result from this stop-work order and will provide follow up information to the Board once com-

REC 0t9ENDATION FOR NOTIFICATION OF LICENSING BOARD In accordance with present NRC procedures regarding Board Notifications, the following information is being provided as constituting new information relevant and material to the Midland ON/0L proceedings. This information deals with Babcock and Wilcox's (B&W) October 5,1983, decision to stop all o.f its Class I (safety-related) pipe hanger and snubber activities. The stop-work applies to both small bore and large bore piping and has resulted in the lay-off of 132 craft and support personnel. The pertinent facts that relate to the stop work are as follows:
1 sampling of B&W pipe support hangers by the Bechtel Corporation, the Midland constructor, identified various discrepancies in the manufacture and installation of the hangers. Bechtel notified B&W, and it in turn took a sample of about 40 hangers for reinspection. B&W found a number of deficiencies, including welding and bolting problems during the inspection.
Both Bechtel and the licensee said that the stop-work order will not be lifted until they have approved a B&W employee training program, and that the appropriate personnel have been retrained. They also will not lift the stop-woth until corrective action is proposed by B&W to resolve any deficiencies in the hanger supports.

BOARD NOTIFICATION ON LIFTING OF STOP WORK ORDERS AND REGION III STAFFING CHANGES ON MIDLAND PLANT (BN 84-083)
In accordance with the NRC procedures for Board Notifications, the following information is being provided directly to the Comission for l' formation.
n This information is applicable only to the Midland Plant, Units 1 and 2.
The information is material and relevant to quality assurance / quality control issues before the Licensing Board in the OM-OL hearing.
The appropriate Boards and parties are being informed by copy of this memorandum.
Board Notificiation 83-167, dated October 28, 1983, advised the Licensing Board that Consumers Power Company had issued nine stop work orders following its audit of the design document control system, stopping extensive safety-related activities at the Midland site on October 22, 1983.
BN 84-019 dated February 2,1984, reported the lifting of the stop work order for remedial <nih work, and supplemental BN 84-023, dated February 14, 1984, reported the partial lifting of some of the other stop work orders.
Enclosure 1 notes that the final stop work order has now been lifted and identifies specific final release dates by the independent third-party overviewer (Stone and Webster) and by the Company. Accordingly, this is the final Board Notification regarding the status of the stop work orders.
NRC followup inspections of the Company's Field Change Request / Field Change Notice (FCR/FCN) documentation will be performed later this year and discussed in Region IH inspection reports to be provided to the Licensing Board and hearing parties.

RECOMMENDATION FOR NOTIFICATION OF LICENSING BOARD'-
FCR/FCN STOP WORK STATUS AND NRC MIDLAND STAFFING CHANGES In accordance with oresent NRC procedures regarding Board notifications, th9 following information is being provided as constituting new information relevant and material to the Midland OM/0L proceedings.
Design Control Stop Work Status On January 31, 1984, Region III notified you of the status of the stop work orders regarding the design control (FCR/FCN) problems at Midland.
On February 14, 1984, a Supplemental Board Notification (BN 84-023) forwarded this information to the Hearing Board.
Following this notification the licensee lifted the remaining stop-works by area engineering discipline.
On March 23, 1984, at 7:30 a.m., the final stop work was lifted.
Attached you will find a copy of a matrix developed by Consumers Power Company that identifies key dates for various releases including Stone and Webster releases and CPCo final release.
As a result of the design control stop work orders and the resultant review of approximately 60,000 documents by Consumers Power Company, 12 nonconformance reports were issued. identifies the noncoriformance reports issued by number, engineering discipline, and a description of the nonconformance.
The licensee has determined that the nonconformances have no significant impact on hardware.
The NRC staf f plans to perforal an inspection on the FCR/FCN area later_ this year to review the new design control system, problems identified by the licensee, and corrective actions taken.
The Board will be informed of the results of this inspection via a copy of the inspection report when issued.
This will complete all required followup action by the NRC.

NRC Staffing Status 't The NRC. staff assigned'to perform inspections at the Midland facility has been increased this fiscal year.
The increase is in accordance with the Region III staffing plan and is due to the backlog of open inspection items and the increased inspection. workload associated with the Construction Completion Program.
The NRC now has three resident' inspectors located at the Midland site, an additional three individuals in the Region.III office assigned full time to the Midland project,'and three inspection specialists from the Region III Division of Engineering who spend time onsite as needed.
Additionally, an NRC Resident Site Supervisor has been selected for Midland and will report to the site in the near future.
NRC is in the process of approving a contract with a national laboratory for assistance with th'e technical inspection program at Midland, approximately i
two and orie-half man yearsi of effort.
In the interim, two Argonne contract engineers, formerly assigned to Zimmer, have been temporarily assigned to provide inspection assistance at Midland.
With the announced decision by the owners of the Zimmer plant t'o not complete it as a nuclear facility, Region III dissolved the Zimmer Section and the Office of Special Cases and transferred the Midland Section together with all its people to the Division of Project and Resident Programs (DPRP).
The
, Director of the Office of Special Cases was reassigned as Chief, Projects Granch 1, DPRP, with responsibility for RIII plants under construction, including Midland.
We believe the increased NRC staffing will help to reduce the backlog of open inspection items and will enable RIII to better monitor the ongoing plant activities.
The organization changes should have no impact on NRC inspection activities at Midland.
No followup actions are required.

SUMARY OF MEETING HELD WITH CONSUMERS POWER ON AUGUST 4,1982 -
MIDLAND PLANT, UNITS 1 AND 2 On August 4,1982, the NRC staff met in Bethesda, Maryland with " Consumers Power Company and Bechtel Power Corp. to discuss use of anchor bolts in masonry walls at. the Midland Plant. This subject relates to section 3.B.2 l
of the Midland SER. A list of. meeting attendees is attached as Enclosure 1.
SUMMARY
The NRC staff is preparing Supplement No. 2 to the Midland Safety Evaluation Report (NUREG-0793). Since issuance of the SER in May 1982, Consumers Power has supplied the additional information needed by the staff to complete evaluation of masonry wall usage at the Midland Plant. This item was listed as confirmatory issue No. 3 in section 1.8 of the SER. The additional information transmitted by a Consumers Power letter dated July 23, 1982, included a Bechtel report on testing of concrete expansion anchors installed in concrete block walls (Bechtel. Report No. 7220-CTR-1-1). This report resulted in design recommendations regarding installation of grouted and wedge-type expansion anchors in grout-filled concrete masonry walls. Bechtel made a presentation to the staff on the purpose and approach used in this testing, using the above report as the basis for discussion. Application and installation criteria / practices were discussed in detail. An anchor bolt of the type used at Midland was used during the discussion for demonstra-tion purposes. The effects of embedment length, anchor size and installation torque were also discussed.
After asking a nunter of questions, the staff concluded that a sufficient amount of information has probably been provided to permit completing the safety evaluation in this area. The staff intends to publish its conclusions regarding the masonry wall evaluation in Supplement No.' 2 to the Midland SER.
