1335 CST Identification and Description of Occurrence:

During the performance of SI-130.1, the turbine-driven auxiliary feedwater pump failed to develop the required operating speed.

Conditions Prior to Occurrence:

Unit 2 Mode 3 (0% power, 547 degrees F, 2235 psig)

Apparent Cause of Occurrence:

The turbine-driven auxiliary feedwater pump developed only 1300 RPM (rated at 3970 RPM). Upon investigation the flow indicating controller (2-FIC-46-57) was found to have the signal input wire incorrectly terminated.

A review was made of previous work done on the controller, and maintenance request no. 163431 was being worked prior to this event by maintenance personnel. The most probable cause for the leads being incorrectly terminated is that a mistake was made in reterminating the wires while trying to locate a ground fault problem on this maintenance request.

e Analysis of Occurrence:

The turbine-driven auxiliary feedwater pump could not have developed the required operating speeds, thus the required flow rate would not have been obtained.

Corrective Action

The wires were correctly reterminated and SI-130.1 was successfully run, with the pump developing the required operating speed. The pump was declared operabic at 1S15 CST on 12/21/81.
