"Zack reported that certain work (fabrication of. duct work and hangers) at That is,the the,ir satellite shop was controlled by a dual traveler system.
traveler that followed the work through the fabrication process was duplicated af ter the work was finished to provide unabused or clean records.
Zack reported that (1) they found an employee was removing the original records f rom their (Zack) record system (2) that examination of the original records (secondary travelers \\ showed that in some cases the weldors identified on the original (secondary record) were dif ferent from those on the primary (copied) traveler.
(3) It appeared that the weldors on the original record may not have'been p:,perly certified to weld at the time the work was done.
f (4) The weldors identified on the prittsry traveler (the copy used for official records) appear to be properly qualified. (5) This dual traveler record system seas used only by the satellite Zack shop. It was not used at the l
l main fabrication shop.

(2) To extent possible, verify which primary travelers identify weldors I
who were not on the original traveler (secondary traveler).
(3) Review all previous CECO audits of Zack which address weldor qualification and certification.
(4) Reexanine weldor qualifications and certification for all weldors identified on travelers. Determine if weldors were properly certified at time work was done.
(5) Establish the adequacy of all documentary evidence attesting to the quality of the welds regarding welding qualification and certification.
(6) ' Eiaborate on reasons why this discrepancy was not determined by prior audit and inspection activities by q11 parties involved.
(7) " rake any othe'r actions dictated by their findings and (8) Make presentation of entire findings, corrective action, and summary to R111 representatives as soon as possible (Mon' day or. Tuesday, first week August 1982).

However subsequently, the overview inspection included only onsite welding, (See second item attachment "A" table #3 from CE.)
In general, in 1979, supports (materials and welds) were given 100%
overinspection by CE.As confidence increased that the process was in control of Zack overview inspections were reduced to 50%, 25,%
and 10% where it is today.

Beginning in April 1980, duct work from'Zack. entering the site (Receipt I,nspection) was 100% overinspected by CE, including V.T. by velds.

Weldor qualification ver'ification was done by CE through routine audit activity. That is, the veldor identified on the available traveler would be traced back to welding and weldor qualification and certification records. CE reports that they.only review "prfmary travelers from the satelite shop" and did not include a review of seconday (original shop) traveler.

Pittsburg Testing Laboratory PRL qualiified and certified Zack Co.
weldors.

Conam did overview inspections for CE.

CE conducted its an audits of Zack Co.

As of Friday July 29, 1982, CE hAd identified:
(1) 24-32 weld joints whos weldor history was discrepant.
(2) Review of about 104 weldors at Zack shows that all are currently qualified and certified.
(3) Four weldors have been identified who may not have been properly certified when the work was done as represented by existing records.
