Narrative

Report for LER 50-321/1981-003 With the tinit at steady state 80% thermal power, 1956 MWt, a llPCI quickstart was to be performed for testing purposes (tune

'< and reduce quickstart transient).

When the test began the llPCI governor valves stuck in the 3/4 open position.

The IIPCI system isolui.ed on a high dP ' trip.

Redundant safety systems RCIC, core spray, ADS, LPCI, and RilR were all operabic.

There wcre ao effects upon public health apd safety due to this

The cause of the governor valves sticking open was one of the two lift rods (piece #58201, part #64 on vendor dwg. 66359E) was bent and galled.

U, The lift rod was bent by the lift rod fork connector (pr;t

  1. 87087C) hf.tting the valve IcVer (part #74 on drawing 66%9E) when the valves are 100% open.. This interEcrence restricted
- the movement of the form in the 100% open valve position and forced the lif t rod away from its true travel planc causing it to bend.

While the rod was bent it rubbed against the upper guide bushing (piccc #58391, part #87 on vendor dwg. 66359E) and caused the surface to be galled.

These two conditions, bending and galling, caused the seizure of the lift rod and thus the stuck open valve condition.

The damaged lift rod was replaced, the upper guide bushing was honed to remove any burrs, and the area on the lift rod fork connector, where the interference occurred with the valve icver was ground away to allow freedom of movement - for all valve positions.

This is a nonrepetitive occurrence and was reportabic under.

Tech Specs section 6.9.1.9.b.

Unit 2 IIPCI turbine was inspected for any similar interference and no indications.of this were found, The unit is now in full compliance, and no further reporting n
