Narrative

Report for LER 50-321/1981-001 On December 28, 1980, with the unit in steady state operation at 1885 etat, HPCI Steam Line Pressure Switches,1E41-N001 A,'C, 0 actuated at 104, 105 and 102.5 psig respectively. Tech Specs table 3.2-2, Item 10, requires actuation of 2100 psig +7.3 psig static head.

Failure of the switches was not reported promptly as required per Tech Spec Section 6.9.1.8.e.

When j

the event was discovered on January 5, 1981, deviations were reported on the failure of the switches and also the failure to report them promptly.

At the time of the event, redundant ECCS systems, RICI, ADS, LPCI and Core Spray were all operable.

Plant operation was not affected. The safety and health of the public was not affected.

This event is repetitive as last reported on LER No. 50-321/1980-114.

The cause of the event was switch setpoint drift.

The Barksdale Pressure Switches were re-calibrated immediately upon discovery of the failure on December 28, 1980 and returned to service.

Personnel were re-instructed Q

to submit deviations promptly.

A generic review revealed no inherent problems. A design change request has been initiated to replace these switches with an Analog Trip System.

This type switch is used in this application on Unit II.

Unit II is presently in refueling and these switches will be checked prior to start-up.
