Narrative

Report for LER 50-321/1980-123 On December 19, 1980, with the reactor in the shutdown mode, the RCIC System was being used to control reactor water level following a reactor The RCIC Turbine was started and stopped successfully three times, scram.

but on the1 fourth attempt, the Trip and Throttle Valve failed to open.

Then on December 26, 1980, during normal operation, while performing the Turbine Exhaust Pressure Instrument Functional Test and Calibration, the Turbine Trip and Throttle valve failed to trip. The RCIC System was declared inoperable after each incident per Technical Specification 3.5.E.2.

After the first failure, the Turbine Trip and Throttle valve was disassembled and inspected.

Gall marks were found on the valve poppet and valve body which had caused the valve to stick in the closed position.

After the gall marks were removed and 'the valve reassembled, the valve was returned to service on December 25, 1980. The second failure was caused by a limit switch having been set incorrectly during reassembly of the valve after the first failure. The limit switch being set incorrectly a owed the valve to overtravel in the open direction causing a mechanical bind which prevented the valve from closing when the trip solenoid was energized.

The limit switch was reset and the RCIC proven operable on December 26, 1980.

j Although the failure of tic Trip and Throttle Valve is repetitive as last reported on Reportable Occurrence Report No. 50-321/1979-02, this was the first event in which the Trip and Throttle Valve failed to open.

This was also the first incident in which a limit switch caused the Trip and Throttle Valve to fail.

After reviewing the maintenance history on the RCIC Trip and Throttle Valve for both units, the nature of this failure does not appear to have any generic effect on the Unit II RCIC Trip and Throttle Valve.

There were no effects to public health and safety as a result of either incident.
