While performing this test the l&C technician observed that the Test / Operate switch for each of the Containment Atmosphere Gaseous Radiation monitors. RM 6123B and RM 8262B, was in the Test position. This switch posioon introduces an artificial 3600 count per minute signal into the radiation monnor making each of the Radiation Monitors inoperable.

The Unit's Technical Specifications require at least one of these radiation monitors for Reactor Coolant System Leak detecuan. These two monnors had been functionally tested on August 23,1965. The switches had been left in the test position at the conclusion of the August funcuonal test. The Unit operated fc 29 days without either of the two gaseous radiauon monitors.

Upon discovery of the switch position the Shift Supervisor was notified and the Test / Operate switches were placed in the Operate position.

determined that two of the Unit's process radiauon monitors were inoperable. Radiation monitors RM S123B and RM S260B are the Containment Atmosphere Oaseous Radmonitors. Technical i-Specihcotions require that at least one of these monitors be operable for the purposes of Reactor Coolant System Leak Detection. The radiation monitors were inoperable because the Test / Operate switch on each morutor was in the Test position. The Test position introduces an artificial 3600 count per minute signal into the radiauon monitor and does not process the signal coming from the detector.

An Instrument and Controls (l&C) Department Technician performing a functional test on one of the Unit's process radiation monitors, observed that the Test / Operate switch on two adjacent radiation monitors, RM $123B and RM $262B, was in the Test position. The 1&C Technician notified the Shift Supervisor who then instructed the technician to place the switches in Operate position. The radiauon monitors were then determined to be operable.

11. Caine nf Frent A review of the past data on these two Radiauon monitors shows that the Test position had been selected during the functional test of these instruments on August 23, 1966. The Operate position was not selected at the conclusion of the functional test. . This resuhed in the instruments displaymg a constant 3600 counts per minute. The root cause of the event being operator error on the part of the Instrument and Controls technician who periormed the functional test on these monitors on August 23, 1968. The restoration step of the functional test, SP 2404AK, was not performed.

These two radiation monitors are observed during the performance of the Operations Department daily surveillances. The surveillance on this monitor did not pick up this discrepancy due to the 3600 count per minute reading being within the normal range for the instrument.

Ill. An A+ of Event This event is being submitted pursuant to 10CFR50.73(a)(2)(i)(B). Technical Specifications require a minimum of one of these two monitors for the purpose of detecting pressure boundary leakage in the Reactor Coolant System. If and when neither of these two monitors are available, grab samples satisf) the requirements of this Specification. Since these monitors were inoperable and grab samples were not taken, the Unit operated outside the requirements of this Specification for 29 days.

The event has no safety consequences. During the period when these monitors were inoperable the leakage limits on the Reactor Coolant System were not exceeded, as determined by the process computer unidentihed leak rate calculation. The two remaining Contamment Atmospheric paruculate radiation monitors and the Containment Sump level indication require;l by this Specification were operable and showed no abnormal condition.

The two inoperable radiation monitors supply a signal to the Engineered Safeguards Actuation Signal, ESAS Cabmets. This signal is an input to a one out of four logic which causes tre closure of the Containment Purge Isolation valves. Since the Purge valves are required to be closed in Mode 1, the inoperabihty of the monitors, RM 5123B and RM F262B had no impact or, the ESAS.

In the event that the Unit had shutdown and cooled down during this 29 day time frame there would still have been no impact as a result of this switch misposition. Unit procedures require manipulation of the Test / Operate switch in preparation for initiation of Conta nment purge. This manipulation would have found and corrected the switch misposition prior to the requirements for its use in the ESAS actuation logic.

to place the Test / Operate swltches for the two radiation monitors in the Operate position. The

! indications which are provided by these monitors was observed to be normal. i The importance of a thorough performance and completion of all surveillances and functional tests was highlighted at the last Instrument and Controls Department meeting. The procedure for the i radiation monitors' functional testing has been reviewed. The procedure data sheet has been

' revised to include a dual verification for documentauon of proper restoration. As a final action to l l prevent recurrence, the Unit has reviewed a change in the type of switch used for the Test / Operate g switch. A spring return to Operate switch has been installed.

The radiation Monitors were manufactured and supplied by Nuclear Measurements Corporation.
