On November 27, 1990, with the plant at 1009 power, the Operations Department determined the action purements of LCO 3.1.2.4, to have at least two charging pumps operable and LCO 3.5.2.d, for an operable rymg pump and flowpath, were not satisfied. LCO paragraph 3.0.3 was entered at this time.

, Action was immediately taken to restore the B charging p. imp to the header within one hour. There was no power reducuon required because the B charpmg pump was restored to operable status within 56 minutes of system isolation. There were no automatic or manually iniuated safety system responses as a result of this event.

The root cause of the event was poor communications. hianagement originally authorized the work but did not realize the extent of the chargirip system tag out required for one of the charging system tasks. Initially,

[ charging and letdown were only to be isolated to reduce exposure, The Operations Department recognized the l requirements of LCO 3.1.2.4 and 3.5.2.d were not satisfied but not until the tags were hung and the system l isolated.

However, while the system isolation was m p ropen, a reexammanon of all anociated Technical Specificauon LCO's revealed that the utm tw nm satsfy the requirements of all sections, and the action requirements of secuon 3.0.3 ucrt enime This was based on not meeting the Technical Specificanon requirements for an opert.bk charg.np flow path. This condition requires the plant to '

commence a shutdown if not corrected wah:r one hour, and steps were taken immediately to restore equipment. 'Withm 56 minutes the synems aere returned to operable status by restoring a charging flowpath, after clearing tags and ven!png preper vahr 1meups, and the unit exited from the acuans of section 3.0.3, There were no automouc a: manually mulated safety system responses as a resuh of this event.

Technical Specifications." The plant complied with the Technical Specifications at all times dunng this event. The loss of all charging occurred for a period of 56 minutes. During this time, prenunter level was maintained within the allowable operating band.

Negative reactivity control was available curmp the time of charging header isolation by an operable rod ct.ntrol systemc There were no safety consequences as a result of this event.

IV. Corrective Action Management has reviewed this evem utv) the events leading up to this incident and has concluded that increased awareness with respect to pem status must be pursued at.the daily planning meeting. This will be acusely exercised in the futun to ensare clear and concise communications are maintained between all departments.
