During normal operation and during the course of ISE Inspection 79-14, it was noted that certain instrument root valves had not been verified in the proper position prior to the plant startup initiated on September 3,1979 Investigation revealed that the procedures intended to provide the verifi-cation of proper instrument root valve positions had been changed in accordance with the plant's approved administrative controls prior to the initiation of the reactor startup. As a result of the temporary change to these procedures a number of valves were deleted because the individuals making the changes to the procedure erroneous 1y understood that verification of the valve position was accomplished by a seperate valve lineup conducted by another department.

These actions re-sulted in a breakdown of the administrative controls required by Technical Specifi-cations Appendix A, Paragraph 6.8.

Double verification of proper instrument root valve position was immediately initiated and the plant was maintained in a steady state operating condition until this double verification could be completed.

In addition, the valve lineup proce-dures fo r i ns t rumen t root, valves of both the Operations and Instrumentation and Control Departments were compared and checked against each other as well as various plant drawings to provide assurance of a complete and accurate verification. The completion of these checks indicated that, with one exception, the instrument root valves were properly posi tioned. A non-safety related Quality Assurance Category 11 valve associated with a pressure switch which monitors pressure between two motor operated valves in Core Spray System "A" was the only va!ve found out of its normal position. The instruments purpose is to alert the operator (via an annunciator) of a high pressure condition which may indicate valve leakage.

Since the double verification of proper valve position was completed and no signi ficant deviations were found, the event did not represent any signi ficant hazard to the public health and safety.
