Dear Mr. Keppler:

On June 30, 1977, we reported to you an event which occurred at the D. C. Cook Nuclear Plant Unit 1 on that date. In that report we stated that we had made a routine release of radioactive gas from the waste gas holdup tanks without having automatic closure for the gas discharge valve in violation of Appendix "B" Technical Specifications Section 2.4.3.d.

The waste gas holdup tank had been properly sampled and approved for release. The vent stack radiation monitors were operating during the release.

This event occurred as a result of a procedural error. The automatic trip function for the waste gas discharge valve RRV-306 had been shifted from R-21 to R-26 when design change RFC DC-12-1384 was implemented. Operating Procedure OHP 4021.023.002 Release of Radioactive Gaseous Waste From Gas Decay Tanks, was revised when the RFC was implemented and the error occurred when the requirement to check R-25 was not included. The pump which supplies R-25 and R-26 must be operable for these radiation monitors to properly monitor the gas release and provide the trip function.

The pump and paper drive for R-25 and R-26 had been disabled to perform repairs.

This RFC eliminated the need to use R-25 as a particulate monitor.

OHP 4021.023.002 Release of Radioactive Gaseous Waste From Gas Decay Tanks, was revised when the RFC was implemented but we failed to include in the revision the requirement to check R-25 to make sure the sample pump is running. The sample pump also supplies R-26.

To prevent recurrence of this event, a temporary change was written to OHP 4021.023.002 to include the requirement for checking R-25 to assure the sample pump is operat~ ing pVioj't/to starting a gas release.

There was no hazard to the health and safety of the public as a result of this event.
