Excessive chlorine feed for Plant conditions.

The amount of chlorine applied was a routine amount that, on previous ceca~

sions, had not caused excursions above the chlorine discharge limits.

No dead or distressed fish were observed during the chlorine discharge.

The September monthly average for all samples collected was 26.9 mg/I.

Apparent Cause:

The 9/27/82 excursion resulted from malfunction of the emulsion breaker and oil removal system combined with oil leaks on the K7A main feed pump.

A previously reported (10/1/82) daily sample of 123 mg/l was the cause of the exceedance of the monthly average limit.

The emulsion breaker oil filters were changed on 9/21/82.

The oil absorbent pads in the turbine sump were changed on 9/27/82 and 9/30/82.

A maintenance request for repairs of the oil leak on the K7A main feed pump has been submitted.

Daily observations of the discharge basin showed no visible oil on the receiv-ing waters.

A discharge concentration of 23 mg/l represents.39 gallons of oil discharged to the Plant's mixing basin based upon a typical daily average of 17,000 gallons from Tank T-41..

Same as identified for individual occurrences PAL-82-07-AD72, AD73 and AD74 submitted on July 22, August 2 and August 6, 1982, respectively.

The -turbine sump oil separator tank (T-41) was pumped out on 7 /28/82 -to remove accumulated oil from the water surface.

The oil absorbent pillows were also changed and the floating oil removal devices in T-41 were emptied.

The daily observation of the discharge showed no visible oil on the lake surface.

Samples collected 7/19/82 and 7/21/82 measured 11.6 mg/l and 13.2 mg/l, respectively.

The oil appeared to be turbine lube oil.

Apparent Cause:

No particular event could be identified as the cause.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

The oil absorbent pillows present in Tankl"-41 and the turbine sump were changed to better absorb any oil present in those tanks.

Additional Comments:

Daily observation of the discharge showed no visible oil on the lake surface.

The oil treatment facility filters were changed on 7/16/82.

The normal expected "life of the filters is greater than four days.

A routine sample collected on 6/28/82 indicated 37.7 mg/liter oil and grease.

Samples collected 6/29 and 6/30 measured 23.1 and 20.8, respectively.

The oil appeared to be turbine lube oil.

Apparent Cause:

Malfunction of oil treatment skid and associated equipment, :possible clogging of oil* removal filters.

If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Oil removal filters changed immediately upon receipt of the 6/28/82 sample results (7/1/82).

Additional repairs scheduled for oil removal equipment in-cluding filter.Ll pressure indicators for showing filter clogging.

Flow surges through the system caused by automatic pump control failures may have con-tributed to the high sample results on 6/28/82.

The pump controls were

On July 2, 1982* the lii:ne remaining on top of the soil after soaking (l"-2i*) was compacted into the soil.

After the compaction the spill area was covered with 6" of topsoil.

The repair of the alarm and the flange was made to prevent recurrence.
