Date of Incident: 9/20/82 2.
Identification of specific effluent limitation or monitoring requirement affected:
011 and grease are limited to 20 mg/l daily maximum concentration.

Description of Incident:
A routine weekly oil and grease sample collected 9/20/82 measured 123 mg/1.
Notification of the analysis results were received on 9/23/82 - sample collected at that time appeared to be clear (within limits).

Apparent Cause:
Malfunction of the emulsion breaker system (oil filters).

Corrective Action - The following corrective action was taken or will be taken to reduce, climinate and prevent recurrence:
The emulsion breaker oil filters were changed on 9/21/02. The floating oil skimmers in the turbine sump oil separator tank were emptied on 9/22/82.

Additional Comments:
Daily observation of the final discharge point inrlicated no visible oil on the receiving waters.
