Identification of specific effluent limitation or monitoring requirement affected:
4 011 and grease monthly average concentration limit of 15 mg/1.

Description of Incident:
During the month of July 1982, the daily maximum limit of 20 mg/l oil and grease was exceeded six days which resulted in the monthly average limit of 15 mg/l being exceeded (average value equals 18.2 mg/1).

Apparent Cause:
Same as identified for individual occurrences PAL-82-07-AD72, AD73 and AD74 submitted on July 22, August 2 and August 6, 1982, respectively.
