REQUEST FOR ADDITIONAL INFORMATION ON ELECTRICAL ISSUES - CABLE AMPACITY PROGRAM, CABLE INSTALLATION PROGRAM, FUSE PROGRAM AND THERMAL OVERLOAD PROGRAM -
BROWNS FERRY NUCLEAR PLANT, UNIT 2 (TAC 62260)
The staff has determined that additional information is required in order to complete the safety evaluations for the subject Browns Ferry Nuclear (BFN) electrical issues.
This formal request for additional information, except for fuse program procedures, is to document earlier verbal requests for clarification of TVA material submitted and TVA explanatory statements made during an audit of BFN Ampacity and Cable Installation Programs.
The information requested is enumerated in Enclosures 1 through 4.
Please provide your responses no later than December 9,
1988 for items identified in Enclosure 2 through 4.
For Enclosure 1 (Cable Ampacity), please provide a
schedule for submitting test results.

ENCLOSURE 1
CABLE AMPACITY PROGRAM By letter dated September 30,
- 1988, TVA submitted responses to the staff's request for additional information on the cable ampacity program.
The staff subsequently discussed the issue with lVA during an audit of the cable installation issue.
It was reported that TVA is relying on the information obtained from flammastic derating and load diversity test programs for assessing cable ampacity.
In order for the staff to complete its review, TVA should submit the above test programs together with the test results and explain how the test results will be used for assessing cable ampacity.
ENCLOSURE 2
CABLE INSTALLATION PROGRAM In order for the staff to complete its review and issue an SER on the Cable Installation Program, the following information is required:
(a) Provide details of the testing methodology and sampling methods to be used by TVA to resolve the concerns identified for cable bend radii and vertical supports.
(b) In order for the staff to conclude the similarity of cable materials at Browns Ferry and Sequoyah, provide a table for cable materials used at Sequoyah similar to the one provided for Browns Ferry in Table 1 of Materials Evaluation and Comparison reports (880714S128).
(c) Cable Issues Walkdown Report (880714S0129, page 7 of 22) identifies that three cuts of 1/2 to 3/4 inch long of indeterminate depth were observed at one pull point and evidence of abrasion and scuffing was found on other cables.
During the walkdown at Sequoyah except for minor abrasion or scuffing of cables no other evidence of cable damage was observed.
Explain and provide,justification for the TVA conclusion that BFN cable installation is enveloped by Sequoyah.
ENCLOSURE 3
FUSE PROGRAM TVA has prepared as requested, one sample of the wiring diagrams showing
- fuses, together with the fuse tabulation corresponding to the sample drawing.
In order to understand the per'tinent attributes of the entire fuse replacement
- program, OSP requests the following additional information:
The total number of fuses on tabulation; 2.
The number of fuses documented during walkdown; 3.
The total number of fuses which did not have to be replaced; 4.
The total number of fuses replaced; 5.

The OSP staff understands that motor control center fuses (MCC) were omitted in fuse tabulation.
Please confirm whether or not MCC fuses are omitted from fuse tabulations and if so, provide the reason for such omission q
A copy of the Browns Ferry Nuclear (BFN) procedure for implementation of the Electrical Fuse Tabulation; and 7.
A copy of the BFN Administrative Instruction defining the requirements for controlling procurement; installation and maintenance of electrical fuses.
ENCLOSURE 4 THERMAL OVERLOAD PROGRAM In order for the OSP staff to complete the safety evaluation for thermal
over load protection at Browns Ferry, the following in ormation is needed.
1.
The total number of thermal overload elements (TOLs) documented during plant walkdown.
2.
The total number of TOLs that did not require replacement or adjustment as a result of walkdown.
3.
The total number of TOLs which required adiustment only as a result of the walkdown.
4.
The total number of TOLs which required replacement as a result of the walkdown.
