This supplement provides updated information on the cause and corrective actions of the Technical Specification required shutdown due to the expiration of LCO time limit, includes revised Energy Industry Identification System (EIIS) codes and reflects minor editorial changes. The January 31, 1995 supplement submittal date was extended to July 31, 1995, by Howard Wong, Branch Chief, Reactor Projects (Telecon January 6, 1995) and was confirmed in letter 102-03230-JML/BAG/RAS dated January 25, 1995. In accordance with 10CFR50.73(d), a copy of this LER. is being forwarded to the Regional Administrator, USNRC Region IV.
