Dear Sirs:

Subject:

Palo Verde Nuclear Generating Station (PVNGS)

-Units 1,-2, and 3 Docket Nos. STN 50-528, 50-529, 50-530 License Nos. NPF-41, NPF-51, NPF-74 Licensee Event Report 93-011-01 File: 94-020-404 Attached please find Supplement 1 to Licensee Event Report (LER)93-011 prepared and submitted pursuant to 10CFR50.73. This supplement reports that an additional undervoltage scenario has been identified that could result in the possible double sequencing of safety-related equipment. That is, following an emergency, safety features actuation, there exists the potential to not only start sequencing safety-related equipment on to the preferred offsite power, but to.load shed due to the Class 1E 4.16 kV undervoltage relays dropping out and not resetting, and then resequencing the equipment on to the emergency diesel generator.

ln accordance with 10CFR50.73(d), a copy of this LER is being forwarded to the Regional Administrator, NRC Region IV. If you have any questions, please contact Burton A. Grabo, Section Leader, Nuclear Regulatory Affairs, at (602) 393-6492.

Sincerely, JML/BAG/KR/pv Attachment cc: L. J Callan (all with attachment)
