Unit 2 Docket No. STN 50-529 Ucense Event Report 94<050, Supplemental Response File: 9520-404 Licensee Event Report 94-005-00, submitted November 18, 1994, indicated that a supplemental report was expected to be completed by January 31, 1995. The results of the initial investigation into the events described in Licensee Event Report 94-005-00 have, however, necessitated additional off-site vendor testing in order to conclusively determine the root cause of the component failure. Due to this additional testing, Palo Verde will not be able to submit the LER supplement as expected.
The revised action plan for completing the root cause of failure evaluation includes:
Submitting the failed components and initial test data to Borg Warner to perform clearance/tolerance calculations.
Repeat the previous testing with a new stem/disk assembly, modifying some of the initial test parameters.
investigate the possibilities of a power circuit phenomenon which may be limiting current flow to the motor actuator.
The additional testing and investigative actions are expected to be completed by July 31, 1995, at which time the results will be communicated to the NRC in the form of a supplemental report to LER 94-005-00. The results of the initial investigation, as well as the need for additional off-site testing, were verbally communicated to Howard Wong, NRC Region Vl, Walnut Creek Field Office, on January 6, 1995.
