Licensee .Event Report 92-013-00 File'2-020-404 Attached please find voluntary Licensee Event Report (LER) 92-013-00 prepared and submitted pursuant to guidelines contained within 10CFR50.73. This LER reports the results of an 'equipment root cause of failure evaluation of economizer feedwater isolation valve (FWIV) 4-way valves. The investigation was completed on October 15, 1992. Based on the results of the investigation, the cause of the FWIV 4-way valve failures was-determined to be due to inadequate vendor supplied components, specifically, due to improper machining, improper manufacturing, and less than acceptable vendor dedication program. The vendor and manufacturer have concurred with the equipment root cause of failure analysis and the vendor has advised the NRC and APS of a 10CFR21 geportable incident related to the 4-way valve failures. In accordance with 10CFR50.73(d), a copy of this LER is being forwarded to the Regional Administrator, NRC Region V.
