The initiation was the result of the failure of the trip logic to reset during the performance of a Reactor Building Exhaust Radiation Monitor surveillance procedure. The'trip logic failed to reset when Plant personnel depressed the reset pushbuttons during the procedure. This caused there to be a half-trip condition in existence and, when Plant personnel continued with the procedure, another half-trip occurred which resulted in the automatic start of the Emergency Filtration System.

An investigation did not reveal the cause of the failure of the trip logic to reset. Plant personnel attempted to reproduce the failure by re-performing applicable sections of the procedure, but the condition could not be duplicated.

There is no safety significance associated with this event as there was no actual initiating condition and all equipment operated correctly to place the Control Room Ventilation System in an isolation condition.

Event Descri tion On June 22, 1987, at 1020 hours0.0118 days <br />0.283 hours <br />0.00169 weeks <br />3.8811e-4 months <br />, a Control Room Emergency Filtration System Fan (WMA-FN-54B) automatically started. The automatic start was the result of the failure of a relay (E-RLY-CR3BX: Division II .Isolation Control Relay - FAZ) to reset during the performance of Plant Procedure 7.4.3.2. 1. 15, "Secondary Containment Isolation Reactor Building Vent Radiation CFT." The procedure provides instructions for performing the Channel Functional Test for Reactor Building Exhaust Air Plenum Radiation Monitor REA-RIS-K609D.

The relay, which provides one trip channel input to WMA-FN-54B, failed to reset when Plant personnel depressed reset WMA Reset Pushbuttons 3BX and 3BY. This caused there to be a half-trip condition in existence and, when Plant personnel continued with the procedure, another relay (E-RLY-CR3BY: Containment I.solation Relay) trip occurred, resulting in the automatic start of WMA-FN-54B.

An investigation did not reveal the cause of the failure of E-RLY-CR3BX to reset. Plant personnel attempted to reproduce the failure by re-performing applicable sections of the procedure, but the condition could not be duplicated.

Immediate Corrective Action After aerification that no actual initiating condition existed, the Control Room Emergency Filtration System was returned to normal lineup.

Further Corrective Action

1. A plant modification document has been prepared to identify the need to include contacts from Relays CR3AX/CR3AY (from RC-1) and CR3BX/ CR3BY (from RC-2) in the RC-1 and RC-2 half-trip annunciator circuits. The modification would provide indication that all-half-trip logic was reset when the half-trip annunciator cleared.
2. Further investigation will be performed in an attempt to determine why the trip logic failed to reset.

Safety Si nificance There is no safety significance associated with this event as there was no actual initiating condition and all equipment operated correctly to place the Control Room Ventilation System in an isolation condition.
