Fate of animal (circle one): dead alive Condition of animal (include comments on injuries, whether the turtle is healthy or emaciated, general behavior while at Salem. At approximately 08:30 a.m., during routine trash rack cleaning, Salem Yard Crew personnel collected a deceased and partially decomposed Kemps ridley sea turtle from Salem 13B circulator intake bay. However, the specimen was not physically observed until 10:10am, after completion of trash rack cleaning and emptying of the north trash basket for disposal. Salem Unit Control and PSEG Nuclear Environmental Affairs personnel were immediately notified. At 10:30am PSEG NEA contacted Environmental Consulting Services Inc. (ECSI), to request assistance with ID verification and to perform specimen processing.

At 10:37am, an additional phone call was made to the NJ Marine Mammal Stranding Center (MMSC) to determine if they would like to take possession of the specimen. Personnel from ECSI arrived on-site at Salem at 12:00pm to complete the required data collection (length and weight measurements, and ID tag checks). PSEG NEA also contacted the NMFS at 10:51am, as well as New Jersey DEP at 10:52am, to inform the agencies of the incidental take. Based on the specimens poor condition, MMSC declined to take possession, and the NMFS therefore directed that the specimen be disposed of after completion of the required processing.

2 APPENDIX B, continued (Incident Report of Sea Turtle Take)

Draw wounds, abnormalities, tag locations on diagram and briefly describe below.

Description of animal:

At the time of observation at 10:10am, the specimen was determined to be deceased, confirmed by evidence of moderate decomposition. Except for a minor skin abrasion to the left hind fin and some missing carapace scutes (refer to illustration), there was no evidence of external injury to indicate a cause of death. Based on the poor condition of the specimen, it appears that death occurred at a much earlier date. No external tags were observed and no internal PIT tags or wire coded tags were identified.

3 Incidental Report Sea Turtle Take -Salem Attachment 1 Dorsal view of the deceased Kemps ridley sea turtle as it appeared after retrieval from Salem CWIS intake bay I13B by Salem Yard crew personnel, during routine trash rack cleaning on the morning of 8-29-17.

Another dorsal view of the turtle showing the condition of the carapace and the head. Damage to specimen was minor, with some missing layers to the anterior costal scutes on the carapace and a slight skin abrasion on the left hind limb. The carapace includes five costal scutes per side, a distinct characteristic of the species (08/29/2017).

4 Incidental Report Sea Turtle Take -Salem Attachment 1 Ventral view of the specimen, showing the condition of the plastron. The inframarginal scutes were observed to have pores, identified to be another characteristic of the species (08/29/17).

Distinct pores p at base of scutes A close-up view of the plastron, showing the location (red circles) of small pores on the inframarginal scutes [08/29/2017].

5 Incidental Report Sea Turtle Take -Salem Attachment 1 A dorsolateral view of the right side of the head (08/29/17).

Anterior view of the turtle showing the dorsal surface of the head and front limbs Distinct claws were observed on each flipper (08/29/17).
