KEWAUNEE POWER STATION 2006 INSERVICE INSPECTION

SUMMARY

REPORT As required by American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code Section XI, 10 CFR 50.55a, and Wisconsin Administrative Code Department of Commerce, Comm 41.55(3), various inservice inspection (ISI) examinations were performed during the 2006 refueling outage at the Kewaunee Power Station (KPS). The refueling outage took place from September 2, 2006 through October 26, 2006. This letter transmits the lnservice lnspection Summary Report as required by paragraph IWA-6230 of ASME Section XI.

Two separate ASME Section XI inservice examination programs are implemented at KPS. One program is for pressure retaining pipinglvessels and component supports, and the other program is for the metal containment.

The 2006 refueling outage inservice inspections met the requirements of these two distinct inspection program intervals as listed below:

The second inspection year of the first period of the fourth interval for Class 1, Class 2, and Class 3 component inspections were performed. The inspections met the requirements of ASME Boiler and Pressure Vessel Code Section XI, 1998 Edition, 2000 Addenda with implementation in accordance with the KPS Fourth Ten-Year lnservice lnspection (ISI) Program 2004-201 4.

The second inspection year of the third period of the first interval for Class MC components inspections were performed. The inspections met the requirements of ASME Boiler and Pressure Vessel Code Section XI, 1992 Edition, 1992 Addenda with implementation in accordance with the KPS First Ten-Year lnservice lnspection (ISI) Class MC Program 1996-2006.

Serial No.06-748 Page 2 of 3 The following examinations and tests were performed for Class 1, Class 2, and Class 3 components and their supports:

Reactor vessel upper and lower internals examination, Steam generator shell circumferential weld examination, Reactor coolant pump main flange bolting and Number 1 seal housing bolting examination, Class 1 valve bodies examination, Reactor coolant pump flywheel examination, Class 1, Class 2, and Class 3 piping and component supports and hangers examinations, Class 1 and Class 2 steam generator primary manway bolting, pressurizer manway bolting, flange bolting, and valve bonnet bolting examinations, Class 1 system leakage test, and Class 2 and Class 3 system leakage tests.

The following items were examined for the Class MC reactor building containment vessel:

Accessible surface areas, Circumferential weld, Seals and gaskets, Airlocks, and Pressure retaining components.

In addition, the following examinations were conducted in excess of ASME Boiler and Pressure Vessel Code Section XI, 1998 Edition, 2000 Addenda requirements:

Nuclear Regulatory Commission Regulatory Guide 1.14, Revision 1, "Reactor Coolant Pump Flywheel Integrity," dated August 1975.

Nuclear Regulatory Commission Bulletin 2003-02, "Leakage From Reactor Vessel Lower Head Penetrations and Reactor Coolant Pressure Boundary Integrity," dated August 21, 2003.

The following item was examined in accordance with KPS Technical Specifications:

Eddy current testing of steam generators.

Documentation summarizing the inservice inspection activities and results relative to the KPS 2006 refueling outage is provided in enclosures 1-9 of this letter. The final reports for each of these examinations are maintained in the Quality Assurance/Quality Control Records Vault at KPS.

The next refueling outage at KPS is tentatively scheduled for the spring of 2008.

Serial No.06-748 Docket No. 50-305 Page 2 of 2 Dispositioning of Class 1, Class 2, and Class 3 indications has been completed in accordance with the rules of American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code Section XI, 1998 Edition, 2000 Addenda for Fourth Ten-Year Interval examinations, and ASME Boiler and Pressure Vessel Code Section XI, 1992 Edition, 1992 Addenda for Class MC components. Applicable codes, standards, and engineering criteria were used to disposition indications associated with the non-code-required examinations. Recordable indications for Class 1, Class 2, Class 3, and Class MC have been dispositioned as summarized below:

1. Recordable indications were recorded on steam generator 1 B circumferential girth weld SG-W1 0. The 43 separate indications were previously recorded during the 1996 and 2001 refueling outages. A comparison of the length, amplitude, and position of the signal response between 1996, 2001, and 2006 revealed no discernible changes, indicating that the flaws are stable and have not grown in size.

Indications were acceptable either by ASME Boiler and Pressure Vessel Code Section XI, 1998 Edition, 2000 Addenda or by fracture analysis as provided in Westinghouse Electric Corporation WCAP-11476, Revision 3, "Handbook on Flaw Evaluation Kewaunee Unit 1 Steam Generator Upper Shell to Cone Weld," May 1991 and evaluation by Kewaunee Power Station (KPS) during the 1996 and 2001 refueling outages.

2. A recordable indication was recorded on the reactor vessel upper internals upper support plate thermocouple conduit at location C-3. The separated conduit cable was previously recorded in 1995 and 2004. The separated conduit at location C-3 showed no noticeable change in separation and was previously determined to be acceptable under the KPS corrective action program.

3. Visual (VT-3) indications recorded on valve bonnet bolting (3 valves) were repaired, reexamined and accepted by KPS maintenance and inservice inspection personnel and reviewed by the authorized nuclear inservice inspector.

4. Visual (VT-1 and VT-3) indications were recorded on a flange for RCS bypass flow.

The indication involved the actual length of the flange studs versus the length called out on "Quality Part Tag." The indication was evaluated by KPS engineering personnel and found to be acceptable.

5. Visual (VT-2) indications recorded during system leakage pressure tests (30 items) were: (1 ) evaluated and accepted by KPS maintenance engineering, program engineering, or inservice inspection personnel; or (2) repaired, reexamined, and accepted by KPS maintenance engineering, program engineering, or inservice inspection personnel. The 30 items were reviewed by the authorized nuclear inservice inspector.

6. General Visual and VT-3 indication consisting of a 3" x 4" scab type indication located on containment plate 158. This scab type indication was accepted by engineering as a preservice surface imperfection during the 2004 refueling outage.

Reexamination during the 2006 refueling outage revealed no noticeable change in the indication on containment plate 158 and was accepted by KPS structural engineering.

In each case, the occurrence and cause of the limitation was documented. In all cases the rnaxinlum amount achievable was examined.

Witnessing and surveillance of the examinations were conducted by personnel from: the Nuclear Regulatory Commission and Hartford Steam Boiler Illspection and Insurance Company of CT.

RESULTS Examinations resulted with the following Recordable Indications being noted on the basis of procedure recording criteria, which are generally more restrictive than specified 4SME Boiler and Pressure Vessel Code Section XI Acceptance Standards.

1. Recordable Indications were recorded on Steam Generator 1B Circumferential Girth Weld SG-W 10. The 43 Separate Indications were previously recorded during the 1996 and 2001 Refueling Outages. A comparison of the length, amplitude, and position of the signal response between 1996, 2001 and 2006 revealed no discernible changes, indicating that the flaws are stable and have not grown in size. Indications were acceptable either by ASME Boiler and Pressure Vessel Code Section XI 1998 Edition 2000 Addenda or by Fracture Analysis as provided in Westinghouse Electric Corporation WCAP 1 1476 Rev. 3 Handbook on Flaw Evaluation Kewaunee Unit 1 Steam Generator Upper Shell to Cone Weld, May 1991 and evaluation by Kewaunee Power Station during the 1996 and 2001 Refueling Outages.

2. Recordable Indication was recorded on the Reactor Vessel Upper Internals Upper Support Plate Thermocouple Conduit at Location C-3. The separated conduit cable was previously recorded in 1995 and 2004. The separated conduit at location C-3 showed no noticeable change in separation and was previously acceptable under the Kewaunee Power Station Corrective Action Program.

3. Visual (VT-1 and VT-3) Indications were recoded on 1 Flange. The discrepancy in the actual length of the flange studs and the Quality Part Tag was evaluated by Kewaunee Power Station Engineering Personnel and accepted.

4. Visual (VT-3) Indications recorded on Valve Bonnet Bolting (3 Valves) were: Repaired, Reexamined and Accepted by: Kewaunee Power Station Maintenance and Inservice Inspection Personnel and reviewed by the Authorized Nuclear Inservice Inspector.

Condition Monitoring was completed. Ncither steam generator 1 A nor 1 B exceeded any performance criteria during the last inspcction cycle (since spring 2003 inspection). No damage mechanisms were required to be evaluated due to the lack of a degradation nicclianisrn being found during the inspection. Steam Generator Operational Assessment from spring 2003 inspection was concluded to be conservative and no corrective actions were required.

There are zero tubes plugged and zero sleeves installed in both steam generator 1A and 1B. Therefore, effective plugging is 0% (zero) in both steam generator 1A and 1B.
