HopeCreekPRNMUpgrade80107709R0SUP19Page3of18HumanFactorsAssessmentRev.1GrandGulfNuclearStation-GrandGulfhadreportableevent(OE300304,GrandGulfLER2012 005 00)duringplantstartupfollowinginstallationoftheNUMACPRNMsystem.ThiseventwasreviewedforapplicablelessonslearnedpertainingtotheHopeCreekmodification.SusquehannaSteamElectricStation-Susquehan na'smodeswitchdesignissimilartoHopeCreek's.OEfromanAPRMscramrelatedtothisde signwasreviewedforapplicabilityatHopeCreek(OE23468).PeachBottomAtomicPowerStation-PeachBottomhasahistoryoflogiccardfailuresintheir2outof4voters(O E35628,301853,312882,302316,300180).NoneoftheOErevi ewedidentifiedHFE relatedproblemsintheNUMACPRNMsystem.3.2RecognizedIndustryHFEIssuesTherearenorecognizedindustryHFEissuesapplicabletotheNUMACPRNMsystem.NUREG/CR 6400wasreviewedandoneissuelistedisspecifictoNeutronMonitoringsystems:afusefailureinanIntermediateRangeMonitorthatmadethechannelinoperablewithnoannunciation.Asaddressedelsewhere,thecomprehensiveself testroutinesofth eNUMACPRNMsystemmakeanun annunciate dfailureextremelyunlikely.3.3RelatedHSITechnologyBecausethePRNMsystemisinstalledinasignificantportionoftheBWRfleetandextensiveOEisavailableforthesystem,andbecauseoftheminimaloperatorinteractionwiththesystem,nocomparisontorelatedHSItechnologywasperformed.3.4IssuesIdentifiedbyPlantPersonnelTheHopeCreekPRNMprojectteamincludesrepresentativesfromOperations,ReactorEngineering,Fuels,Maintenance,OperationsTraining,andMaintenanceTraining.Becauseofthelimitedinteractionoperatorshavewiththesystem,nohumanfactorsOEwasidentified.ThedesigndecisiontouseOperatorDisplayAssembliesandtoplacethemonthe10C651Operator'sConsolewasbasedoninterviewswithOp erationspersonnelandmock upsoftheproposeddesignintheHopeCreekSimulator.Likewise,thedecisiontomaintaintheAPRMstatusindicatorlightsonthe10C651Operator'sConsolewasbasedoninputfromalicensedoperatoronthePRNMprojectteam.3.5ImportantHumanActionsThePRNMsystemperformsanautomaticsafetyfunctionandtherearenoimportanthumanactionsperformedbyplantoperatorsonthePRNMsystem.ImportanthumanactionsrelatedtocommoncausefailuresofthePRNMsystemareaddressedseparatelyinaccordancewithNUREG 0800Appendix18 A.4FunctionalRequirementsAnalysisandFunctionAllocationThePRNMsystemprovidesanautomatictriptoRPS,andnooperatoractionisrequiredtoaccomplishthesafetyfunction.PerSectionG.1ofthePRNMLTR(NEDC 32410P A):

HopeCreekPRNMUpgrade80107709R0SUP19Page18of18HumanFactorsAssessmentRev.1Preinstallationtraining(1/2018)Thiswillbethefirsttrainingprovidedtoentirelicensedoperatorpopulation.ThistrainingwillbeperformedutilizingthecomputerroomPRNMsand/orthesimulatorPRNMdependingonsimulatorinstallation.Justintimetraining(4/2018)Thistrainingwilloccurduringth eoutagewhilei nstallationisoccurring.Itwillcontainuptodateinfo rmationthatmaynothavebeencoveredinthepreinstallationtraining.Thistrainingwilloccurintheclassroomand/orsimulatoroncethesimulatorinstallationiscompleted.GaptrainingpostoutageThistrainingwilloccurinatrainin gsegmentfollowingtheinstallation(Spring/Summer2018)tocoveranyga psidentifiedbetwee ntheprevioustrainingandfinalstateoftheproject.Maintenancetrainingincludesaninitial"trainthetrainers"in4thquarterof2016,withdetailedtrainingupdatesdevelopedthroughthe1stand2ndquartersof2017.Maintenancetrainingonthenewsystemwillbeginin4thquarter2017toberea dyforsysteminstallationin2ndquarter2018.StartupandinitialoperationswillbemonitoredandanyproceduralortrainingdeficiencieswillbeenteredintoPSEG'scorrectiveactionprocessfortrackingandresolution.EvaluationofthetrainingprocessforthePRNMsystemwilloccur6 12monthsfollowinginstallation.Itwillincludeareviewofsystemperfo rmanceandpersonneloperationofthesystemsinceinstallationtodetermineifanyfurthertrainingisrequiredoriftheinitialtrainingwassufficient.Thisupgradeisnotpartofacontrolroommodernizationprogram.13HumanPerformanceMonitoringStrategyNUREG 0711identifiesthat,"Ahumanperformancemonitoringstrategywillhelptoprovidereasonableassurancethattheconfidencedevelopedbythecompletionoftheintegratedsystemvalidationismaintainedovertime."Asidentifiedabove,withnochangesbeingmadetoanyimportanthumanactionswiththeinstallationofthisreplacementsystem,noIntegratedSystemValidationiswarranted.Sin cethesystemprovidesautomaticfunctions(e.g.,inputtoReactorProtectionSystemscramsignals,inputtotheReactorManualControlSystemcontrolrodwithdrawalblocks),thesameastheexistinganalogNMSsystems,therearenochangesinrequiredoperatoractions.Therefore,thereisnoimpacttoHopeCreek'sexisti ngHumanPerformanceMonitoringPr ogram.
