Licensee Action on Previous Insoection Findinas (0 pen) Unresolved Item (31-01-01):
Concern regarding welding problems:
The inspector was provided for review a copy of the Daniel International Corporation (Daniel) Special Welding Task Force report dated February 1981.
The inspector learned that four significant problem areas had been identified and that 20 separate conclusions were noted with recommendations for corrections of each.
The inspector understood that action to imple-ment the recommerdations is currently underway.
Further review will be conducted in the future.
(Closed) Inspection concern (80-18 and exit meeting of October 2,1980)
regarding non-conical breaks of 90-day concrete test cylinders.
A Daniel investigation was conducted by Cor orate Technical Services regarding r
this concern expressed during an exit interview on October 2, 1980.
A report copy was provided to the NRC inspector dated April 29, 1981, of the results of this investigrtion.
The NRC inspector concluded that the investigation and report were thoroughly done and that the conclusion reached was justified.
The NRC inspector indicated that he had no further questions regarding this matter.

Main Control Room Consoles and Panels Weld Insoection The NRC inspector attended a pre-inspection meeting on April 7,1981 regarding the Bechtel Power Corporation and Comsip Customline Corporation inspectors plan and procedure to inspect the structural welds of the consoles and control panels located in the Control Room.
The inspector was informed that a mockup test panel had been prepared with a range of sizes of fillet welds and that the test panel had then been seismically tested well beyond the seismic load requirements.
It was understood that the console and panel welds would be inspected and compared to like welds on the test panel to establish acceptability and the results then submitted to Bechtel for engineering analysis.
The NRC inspector observed the inspection activity in progress and observed the documentation of weld inspection results on Comsip Drawing No. 5775-9, Revision 10, dated May 15, 1980, " Main Control Board RL021 and RL022 Plan, Rear, and Side Elevations" Sheet 2 of 3.
The NRC inspector had no comments on the inspection activity in progress;
"however, more review of the results of this inspection work is planned in the future.

Safety-Related Pioing Hangers Inspection / Investigation Allegations of improper welding and material control practices on safety-related piping hangers were received by the NRC Duty Officer on April 7, 1981.
Communication errors delayed the actual contacting of allegers until April 9, and April 10, 1981 at which time it was learned that a hanger had actually been fabricated from nonconforming material and had been given the safety-related hanger identity that it was used to replace.
The real hanger (Number 1-BN-01 C011 123) had been installed on March 13, 1981.
This hanger was inadvertently destroyed when it was cut down to adjust for misalignment or, March 14, 1981.
The unauthorized,
,
nonconforming hanger was fabricated on March 16, 1981, placed in the
{
hanger room and documented on the hanger card on March 17, 1981.
The NRC inspector observed this hanger on April 13, 1981 in the Piping l
Superintendent's office along with a remaining piece of the original
'
hanger.
It could be observed that:
(1) the stamping had been crudely done with the letter "B" being formed using number 1 and 3 stamps, (2)
the welder stamp number had been placed on the I-beam flanges next to the welding done, and (3) heat number traceability had not been stamped on i
l the parts.
All of these indicators had been confirmed as the alleger l
had indicated.

The NRC inspector reviewed Daniel procedures for proper material control requirements as follows:
Work Procedure WP-VII-209, Revision 7 " Preparation and Processing of
.
Travelers" requires in paragraph 4.27.a.5 that if material is lost, damaged, or misfabricated, a replacement item may be obtained or the old corrected, in accordance with an ECR.
Replacement items shall have new identification.
The act of fabricating a pseudo hanger from nonconforming material, substituting existing hanger numbers for the required new identifica-tion and stamping false welder identification numbers on the piece is an act in violation of 10 CFR Part 50, Appendix B requirements and has been cited as a violation in a letter to the licensee.

Plant Tours i
One or more plant areas were toured several times during the reporting period to observe general construction practices, area cleanliness and storage condition of plant equipment.
No violations or deviations were identified.
6.
Unresolved Matters Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of non-compliance. or deviations.
An unresolved item disclosed during this inspection is discussed in paragraph 3.
7.
Exit Interviews The RRI met with licensee representatives identified in pragraph 1 to discuss the various inspection findings on April 9 and April 23, 1981.
The RRI attended an exit meeting conducted by Mr. D. D. Driskill, the Region IV Investigation Specialist on May 14, 1981.
