Reactor Projects Section IB Inspection Summary Inspection on April 21-22, 1981 (Reports No. 50-456/81-06; 50457/81-06)

Areas Inspected: Electrical cable installation activities, follow-up of licensee action relative to noncompliance, unresolved matters, items reported pursuant to 10 CFR 50.55(e), IE Bulletins and IE Circulars. The inspection involved a total of twelve inspector-hours onsite by one NRC inspector.

Results: No items of noncompliance or deviations were iden-tified during this inspection.

Procedure 7A, Revision 2, " Post Tensioning Tendon and Component Receiving and Storage" clarifies inspection re-quirements. Procedure 7B, Revision 4, dated September 9, 1979, " Post Tensionin- ~

s11ation" indicates pertinent data to be recorded on the tendon p 1.

All procedures have been reviewed and approved by the his architect / engineer. This matter is considered resolved.

(Closed) Unresolved Item (50-456/79-14-01; 50-457/79-14-01): Licensee observance of " witness points" at vendor facilities. The inspector re-viewed CECO BG-19, Revision 0, dated February 8, 1980, " Inspection Point Frogram". This procedure delineates the requirements for with witness point surveillance. No inspection point can be waived without the con-currence of the site QA organization. All such waivers are required to be documented. This item is resolved.

(Closed) Noncompliance (50-456/80-12-03; 50-457/80-11-03): Storage of safety related HVAC ducts. HVAC ducts have been placed on dunnage in the laydown area. Contractor has implemented a monthly surveillance of HVAC duct storage area to prevent recurrence.

CECO QA has performed two audits and two surveillances in this area since the noncompliance was identified. This item is considered closed.

1978. Final nondestructive examination (magnetic particle) results were accepted by ANI on April 17, 1978. This item is closed.

Electrical Cable Installation the inspector examined Class IE electrical cable activities that had been performed prior to the inspection.

Procedures relative to cable installations, inspection and audit reviewed were:

L. K. Comstock Procedure 4.8.8 (Cable installation Inspection) dated March 12, 1981, was approved by the architect / engineer on April 1, 1981 and by the licensee on April 8, 1981.

L. K. Comstock Procedure 4.3.1 (Safety Related Conduit Installation)

dated July 18, 1980, was approved by the architect / engineer on August 19, 1980, and by the licensee on September 10, 1980.

L. K. Comstock Procedure 4.14.1 (Audit Proredures) dated January 19, 1981, was approved by the architect / engineer on April 1, 1981, and by the licensee on April 8, 1981.

The minimum bend radius was violated during installation of cables IAP077, 1AP078, 1AP323, IAP324, IAP325 and 1AP326. The minimim bend radius for these cables is 19.5 inches as id tified in procedures.

The installers used a 12-inch pulley in one portion of the cable pull.

This violation of the minimum bend radius is documented in NCR No. 97 dated March 30, 1981.

Bulletins and Circulars The inspector reviewed the 1 censee's actions relative to the following IE Bulletins and Circulars.

a.

IEB 80-09 - Closed. The actuators are not on site. The insyector observed documentation in which the licensee instructed the supplier (Masovielis) to replace the deficient springs prior to shipment.

The safety related application of these actuators involves two valves

in the ecsentici service water system to the control room HVAC.

IEB 80-12 - Closed. No respcnse was required to this bulletin.

Written instructions delegating responsibility for review and documentation of actions regarding the bulletin have been issued by the licensee.

c.

IEB 80-23 - Closed. Eight of the Valcor valves identified in the bulletin are to be installed at Braidwood. The eight Valcor solenoid valves with possible defective coils have been returned to Valcor for coil replacement (Valcor PNJ70900-21-1)

IEC 79-10, IEC 80-05, IEC 80-09, IEC 80-10, IEC 80-12, and IEC 80-13 -all closed. Written instructions have been issued assigning responsibility for review, action and documentation of results relative to each of these circulars.

Exit Interview The inspector met with licensee representatives (denoted under Persons Contacted) at the conclusion of the inspection on April 24, 1981. The inspector summarized the scope and findings of the inspection. The licensee acknowledged the information as reported.
