Inspection on January 16-17, 1980 Report Nos. 50-454/80-01; 50-455/80-01) Areas Inspected: Licensee actions relative to the resolution of noncom-pliances cited during the RIII Mid-Term QA inspection of September 11-28, 1979. This inspection involved a total of 14 on-site inspector hours by one NRC inspector.

Results: No items of noncompliance or deviations were identified in the areas inspected.

Licensee Action on Previous Inspection Findings (Closed) Noncompliance (50-454/79-14-01; 50-455/79-14-01).

Inadequate procedures and failure to follow procedures with regard to qualification of inspection personnel. The resolution of this noncompliance is dis-cussed in detail in Paragraph 2 of the body of this report. The noncom-pliance is considered to be closed.

Review of Licensee Actions on Previous Inspection Findings The RIII inspector reviewed the licensee's actions relative to , the resolution of specific inspection findings identified during the RIII Mid-Term QA inspection at Byron on September 11-28', 1979 and reported in RIII inspection report (50-454/79-14; 50-455/79-14).

The items reviewed and the licensee actions relative to the resolu- , tion of the items, are discussed in the following sections of this report.

Qualification and Certification of Inspection Personnel RIII Report (50-454/79-14; 50-455/79-14), which reported the results of the RIII Mid-Term QA Review for Byron, noted three examples of inadequate procedures or failure to follow procedures with regard to qualification / certification of inspection personnel. The three ex-amples are as follows: a.

Commonwealth Edison Station Construction field engineers who jerformed material receipt inspections were not certified in ' accordance with the requirements of ANSI N45.2.6 for inspection personnel.

b.

The procedures for Hunter Corporation, Blount Brothers Corporation, and Hatfield Electric Corporation were found to contain inadequate requirements for the qualification and certification of inspection personnel in accordance with ANSI N45.2.6.

The Hatfield Electric Corporation personnel qualification files did not provide adequate documentation and justification in accordance with ANSI N45.2.6 for the certifications of QA inspection personnel.

During this inspection, the RIII inspector reviewed the following ' procedures relating to the qualification and certification of inspection personnel: a.

CECO Procedure SCD-DOP-NC-9, dated November 30, 1979, "Qualifi-cation and Certification of Inspection Personnel for Inspections and Examinations."

, b.

CECO QA Department Memorandum No. 7, Revision 2, dated November 15, 1979, " Personnel Qualification and Certification Program for Personnel to Meet the Intent of ANSI N45.2.6."

c.

Hunter Coccoration Procedure S.I.P. No. 1.702, Revision 4,

dated January 8, 1980, " Quality Assurance Personnel Selection, Training, ani Certification".

Blount Brothers Corporation QA/QC Work Procedure No. 33, Revi-s sion 3, Issue 4, dated November 8,1979, " Personnel Qualification and Certification."

Hatfield Electric Company Procedure No. 17, Revision 1, Issue 1, dated December 11, 1979, " Qualification of Inspection and Audit Personnel."

- The RIII inspector also reviewed the qualification and certifications ' records for 19 CECO Station Construction Department personnel (16 Level II and 3 Level III), for eight CECO QA Department personnel (7 Level II and 1 Level III), and for eight Hatfield Electric Company personnel (2 "in-training", 4 Level II, and 1 Level III).

Based on the review of the above procedures and personnel records, the inspector had the following specific comments, which were communicated to the licensee: CECO Procedure SCD-DOP-NC-9 refers to minimum requirements a.

for " construction related experience", whereas ANSI N45.2.6 refers to " experience in equivalent inspection, examination, or testing activities."

b.

The Level II certification for the CECO Station Construction Department personnel are marked as qualifying each person for inspections in the structural, mechanical and electrical areas, with no written rationale provided for certification outside of the person's specialty area.

c.

One of the Hatfield Electric Company personnel certified as Level II appeared to lack the experiente to qualify for that level, either under Hatfield Procedure No.17 or ANSI N45.2.6.

The RIII inspector discussed the above comments with the licensee.

The licensee's Site QA Supervisor stated that: a.

CECO Site QA was in the process of conducting an audit of the CECO and site contractors' revised personnel qualification pro-cedures and personnel qualification records.

b.

Comment b. above had been noted during the CECO audit and was being resolved, and c.

Comments a. and c. above would be considered during the CECO QA audit.

Based on the discussion with the CECO Site QA Supervisor, the inspec-tor classified the above comments as an unresolved item (50-454/80-01-01; 50-455/80-01-01); the resolution of which will be reviewed during future inspection.

Other than for the above three specific comments, the inspector's . review of the revised procedures and personnel qualification records indicated that appropriate corrective actions have been taken by the licensee with regard to the previous noncompliance (50-454/79-14-01; 50-455/79-14-01). The noncompliance is therefore considered to be resolved.

. 3.

QA Documentation Control ' RIII Report (50-454/79-14; 50-455/79-14) noted two examples of in-adequate review or coordination of QA-related documentation. The two examples sre as follows: a.

Fourteen CECO. surveillance check sheets involving calibration of instruments used by Hunter Corporation and eleven surveil-lance check sheets involving concrete and grout pour preparation by Blount Brothers Corpo.ation were not properly reviewed and approved by Ceco. QA, as required by CECO.

Quality Procedure QP No. 18-2.

b.

Three Field Change Requests, FCRs 605, 775, and 780 involving Hunter Corporation activities, were not properly logged and coordinated by CECO. Station Construction, as required by CECO.

Quality Procedure QP No. 3-2.

During this inspection, the RIII inspector reviewed the surveillance check sheets related to calibration of instruments used by Hunter Corporation, and concrete and grout pour preparation by Blount Brothers Corporation, and verified that all check sheets during the

periods reviewed had been reviewed and approved by CECO QA.

The RIII inspectoralso reviewed the Field Change Request (FCR) Log maintained by CECO QA, and the FCR files, and verified that FCR's 605, 775, and 780, and ten other FCR's selected at random had been properly logged and coordinated-Based on the above reviews, the inspector concluded that appropriate corrective actions have been taken by the licensee relative to the noncompliance (50-454/79-14-02; 50-455/79-14-02) cited in the previous RIII report. That noncompliance is therefore considered to be resolved.

Unresolved Items , Unresolved items are matters about which more information is required in order to determine whether they are acceptable items, items of noncompli-ance, or deviations. An unresolved item disclosed during the inspection is discussed in Paragraph 2 above.

Exit Interview The inspector met with licensee representatives (denoted under Persons Contacted) at the conclusion of the inspection on January 17, 1980. The licensee acknowledged the findings reported herein.
