Inspection on April 23-26, 1990 (Report No. 50-440/90009(DRS))

l Areas Inspected: Special safety inspection concerning previously identified environmental qualification (EQ) inspection findings concerning::(1) the

licensee's corrective actions regarding the installed configuration of EQ Limitorque Motor Operated Valves (M0Vs) and EQ Target Rock' Solenoid Valves-(SOVs); (2) the licensee's corrective actions to prevent the recurrence of EQ violations; (3) the licensee's corrective actions concerning the submergence of EQ terminal blocks and splices located in electrical enclosures; and (4) the licensee's actions to resolve a Regulatory Guide (RG).1.97 -deviation concerning reactor water level instrumentation (Modules 30703, 25587 and 62705).

Results: This inspection' revealed that the licensee has taken adequate corrective actions to resolve the previously identified EQ and RG 1.97 inspection findings and concerns.

Consequently, no violations, unresolved items, or open items were identified as a result of this inspection.

Strengths observed in the licensee's EQ program included the following:

The licensee has taken steps to improve EQ awareness and communications among the various plant organizations by conducting EQ meetings and training sessions.

Licensee Action on Previously Identified Findings'

(Closed) Deviation-(50-440/89011-01(DRS)):

This deviation addressed the failure of the licensee to comply with Regulatory Guide (RG) 1.97, Revision.2,'in that reactor water-level instrumentation did not meet Category I requirements.

During this inspection, the inspector reviewed-the licensee's response to the deviation. The response stated-that Amendment 18 to the Final Safety Analysis Report (FSAR) dated April 10, 1985,. identified CEI's position that the Category III reactor water level instrumentation does not perform any automatic functions. Therefore, CEI stated that

it was acceptable to deviate' from the regulatory guide. However, this exception was not-identified in the licensee's RG 1.97 submittal and was consequently not accepted as part of the RG 1.97 Safety Evaluation Report-(SER).

Subsequently, NRR informed Region Ill;that the licensee's reactor water level instrumentation was acceptable.

This unresolved item concerned the. licensee's failure-to take adequate corrective actions to resolve a previously identified EQ Severity Level 111 violation.

Subsequent review by the NRC. determined that

this unresolved item represented a Severity Level Ill violation of 10'

CFR 50, Appendix B, Criterion XVI, " Corrective Actions".. Consequently, a Notice of Violation (NOV), was, issued on July 11, 1989,.which addressed the licensee's failure to ensure that environmentally.

qualified terminal blocks or Raychem splices-and!T-drains were

installed in EQ Limitorque motor operated valves (M0Vs) located in harsh environments. The NOV also addressed EQ Target Rock solenoid valves (S0Vs) that were not installed in the qualified configuration

in'that loose screws were found on.the cover housings.

Subsequent to the NRC findings, the licensee took immediate corrective actions and performed-inspections of EQ Limitorque MOVs to ensure that qualified Marathon 300 terminal blocks, Raychem splices and T-drains

,

were installed and that Target Rock S0V cover housing. screws'were tight in order to prevent moisture intrusion.

In addition, thet licensee initiated walkdowns of each EQ equipment type to verify that the equipment was. properly installed.

The results.of these walkdowns'were documented on Surveillance Reports 89-88 and 89-242.-

During this inspection, the inspector-reviewed the licensee's corrective actions to prevent recurrence.. Actions taken by the licensee include the implementation of EQ meetings and training sessions to increase EQ awareness throughout the plant, assurance.

This item concerned 12 EQ terminal boxes located inside containment that did not contain weep holes to allow the drainage of accumulated water during accident conditions.

The licensee stated that the

i terminal boxes were sealed by a.Tremco tape' gasket mateEial, and that the conduits which enter the boxes were configured such'that

they would not serve as a path for moisture to enter the enclosures.

However, the inspector was concerned that the licensee did not have i

EQ documentation on file to demonstrate the ability of the Tremco tape to prevent moisture intrusion or documented evidence concerning

the conduit configurations.

t The licensee performed walkdowns and verified that EQ penetrations-and terminal boxes are installed in the qualified configuration and-that conduits which enter the enclosures would not serve as a path for moisture intrusion.- In addition, the licensee installed weep _

holes in the terminal boxes located in containment. Theilicensee stated that the gasket material is a nonsafety-related material and is not relied upon to prevent moisture intrusion.

Based upon the.

The Region 111 inspector met with licensee representatives (denoted lin Paragraph 1) at the' conclusion'_of the inspection ~on April 26, 1990.-- The inspector discussed the likely. informational. content of the inspection
