Areas Inspected:
Included a review of a security incident involving a vital area (VA) barrier degradation that occurred on June 27, 1986 and the reportability aspect of the incident.
Results: Based on this inspection, two apparent violations were identified:
1.
Access Controls - Personnel:
Licensee failed to adequately control access to a vital area because of an unsecured physical barrier.
2.
Records and Reports:
The licensee failed to report to the NRC a major loss of security effectiveness, uncompensated for, within the one hour

requirement specified in 10 CFR 73.71(c). The event was reported late.
