Exit Interview The inspection scope and findings were summarized on May 25, 1983, with those persons indicated in paragraph 1 above. The violation delineated in paragraph 3.b. was discussed in detail. The inspector indicated that the root cause of the violation appeared to be inadequate communications to the personnel responsible for the installation and inspection of instrumentation and this area should be addressed in the licensee's review of this item.

Licensee Action on Previous Enforcement Matters a.

(0 pen) Unresolved Item (413, 414/83-05-01): Generic evaluation of vendor welding discrepancies. The licensee summarized the results of their extensive review of vendor welding discrepancies on May 23, 1983.

This item remains open pending detailed NRC review of licensee actions in this area.

(Closed) Unresolved Item (413, 414/83-08-01): Verification of adequate control of instrumentation installation.

Further review conducted by the inspector in this area has resulted in a change of this item to a violation. This review verified that the licensee has not installed Unit 1 instrumentation lines in accordance with applicable instructions.

Instructions for installation of instrumentation are contained in standard drawings. Drawing No. 1CS-A-20.3, Rev. 6 requires that functionally redundant safety class instrument lines for nuclear safety related instruments meet the physical separation criteria of 18-inches minimum. During a previous inspection on April 13,1983 the inspector noted that the safety class instrument lines for nuclear safety related instrument Loop Nos. ISM 511 and 1SM514 did not meet the 18-inch separation from their functionally redundant lines. The lines were within several inches of their redundant lines.

The licensee does have a program in place for analysis of instru-mentation line separation in hostile areas which probably would have served to identify this condition at a later date. However, due to a lack of communication in the instrumentation area, initial instal-lation of these lines was in violation of intended separation criteria and was accepted by QC. This misunderstanding could have led to other lines being imprcperly installed initially and lines in non-hostile areas remaining installed without meeting the established separation criteria. This is violation 413/83-12-01:

Failure to follow drawing for installation of instrumentation lines.

No violations or deviations, except as described in paragraph 3.b., were identified.

Independent Inspection Effort (Units 1 and 2)

The inspector conducted general inspection of site work and work activities, observed protection and storage of mechanical equipment, electrical equipment, electrical cables, instrumentation and piping, and observed housekeeping. The inspections were performed in the auxiliary building, containment buildings, and storage areas.

During this _ inspection the inspector noted that weed killer had been recently sprayed in outside storage areas and that this chemical had probably also been sprayed on to some of the materials stored in these areas, including stainless steel materials. The inspector reviewed the licensee program for control of this chemical and determined that the chemical used had been appropriately certified for use on safety related materials, including stainless steel.

No violations or deviations were identified.
